Objective: To research the effect of emergency trauma surgery mode in the treatment of polytrauma.Methods: The clinical data of patients with multiple trauma admitted to Affiliated Hospital of Guilin Medical College from January 2018 to December2020 were retrospectively analyzed.According to the outcome of multiple trauma patients,they were divided into death group and survival group.According to the different treatment mode,they were divided into emergency trauma surgery model group(study group)and triage and division model group(control group).First,logistic regression analysis was used to study the influence of different modes on the death of patients with multiple trauma,and then t test and chi-square test were used to study the application effect of emergency trauma surgery mode in the treatment of multiple trauma.Finally,Kaplan-Meier survival curve was drawn to analyze the differences in outcomes between the study group and the control group.Results: In this study,a total of 116 patients with polytrauma were included.logistic regression analysis showed that emergency trauma surgery mode was a protective factor for patients with polytrauma(OR=0.262,95%CI:0.085 ~ 0.811,P=0.020).There were 54 cases in the study group,including 34 males and 20 females,with an average age of(44.9±14.8)years and an average ISS score of(22.26±8.29)points.There were 25 cases of traffic accident injury,14 cases of falling injury and 15 cases of other injuries.The mean hospitalization time was 16.20±5.24 minutes,and the mean preoperative time was 75.70±16.72 minutes.Among them,28 patients received blood transfusion,and the average time of transfusion determination was 2.72±0.59 hours.The average mechanical ventilation time was 3.19±1.14 days,the average hospitalization time was 16.24±7.17 days,and the average hospitalization cost was 99670.10±40415.47 yuan.The incidence of complications was 13%,mortality within 7 days was 9.3%,and overall mortality during hospitalization was 9.3%.In the control group,there were 62 cases,including 40 males and 22 females,with an average age of(43.9±15.4)years and an average ISS score of(24.53±7.93)points.There were 36 cases of traffic accident injury,15 cases of falling injury and 11 cases of other injuries.The mean hospitalization time was 26.08±10.11 minutes,and the mean preoperative time was 105.08±14.65 minutes.Among them,29 patients received blood transfusion,and the mean time of transfusion determination was2.95±0.47 hours.The average mechanical ventilation time was 4.53±1.53 days,the average hospitalization time was 22.06±6.84 days,and the average hospitalization cost was 125804.33±39231.22 yuan.The complication rate was30.6%.The mortality within 7 days was 24.2%,and the overall mortality during hospitalization was 30.7%.There was no significant difference in gender,age,ISS,cause of injury,and time to transfusion determination between the two groups(P>0.05).The hospitalization time,preoperative time,hospitalization time,mechanical ventilation time,hospitalization cost,complication rate and mortality rate of the study group were lower than those of the control group,and the differences were statistically significant(P<0.05).The comparison of Kaplan-Meier survival curve showed that the survival rate of patients with polytrauma in the study group was higher than that in the control group.Conclusions: Emergency trauma surgery mode can effectively shorten the preoperative time,hospitalization time,hospitalization time and mechanical ventilation time of patients,and can also reduce the hospitalization cost of patients,the incidence of complications and mortality,which has high application value.
