| Objective:Compare traditional scalpel cutting and traditional V shape incision suture,low power power electric cutting and traditional V shape incision suture,low power power electric knife cutting and improved micro inverted trapezoidal incision inverted suture three different cutting and suture surgical treatment to reduce the clinical application effect of thyroid surgery neck scar,in order to provide a suitable cutting and suture method for thyroid surgeons.Methods:This study using prospective randomized controlled study method,select the traditional open thyroid surgery in Nanxishan Hospital of Guangxi Zhuang Autonomous Region from October 2020 to October 2021,it was divided into 3 groups by random number table,group A by traditional scalpel incision and traditional V-shaped incision surgery,group B according to low-power electric incision and traditional V-shaped incision surgery,group C according to low power electric knife cutting and modified micro inverted trapezoidal incision varus suture surgery.All the three groups were completed by traditional open thyroid surgery by the same group of senior doctors according to the quality management standards of our hospital,All study subjects received the same postoperative port recovery treatment,Operation time,intraoperative bleeding,postoperative discharge,discharge of hospital stay,hospital cost,visual analog pain score at 12 hours after surgery(Visual Analogue Scale,VAS),the 36-hour VAS score,and the incidence of postoperative incision complications,The incision healing at 1,6,and 12 months after surgery,Refer to the Vancouver Scar Rating Scale(Vancouver scar score,VSS)and the Patient Scar Assessment Scale(Patient scar assessment scale,PSAS)for the scoring.Through the comparison of the above indicators,the evaluation of traditional scalpel cutting and traditional V shape incision suture,low power power electric knife cutting and traditional V shape incision suture,low power power electric knife cutting and improved micro inverted trapezoidal incision inverted suture three different ways of skin cutting and suture surgery to reduce the clinical application effect of thyroid surgery neck scar.Results:The difference between the number of perioperative time,intraoperative blood loss,postoperative discharge,VAS score at 12 hours after surgery,VAS score at 36 hours,hospital stay and hospital cost was not found in the three groups(P> 0.05).The incidence of postoperative incision complications,such as incision infection,subcutaneous hematoma,and fat liquefaction,was not significant in the three groups(P> 0.05).VSS scores compared between January,6 and 12 months were significant among the three groups(P <0.001).A further pairwise comparison between groups showed the VSS scores between groups A and Group B,Group B and Group C,and Group B and Group C(P <0.001).At 6 months,the VSS scores between group A and Group C,group B and Group C were statistically significant(P <0.001),and the VSS scores between Group A and Group B(P> 0.05).At 12 months,the VSS scores between group A and C were statistically significant(P <0.001),VSS scores between group B and C(P=0.002 <0.05),and VSS scores between Group A and B(P> 0.05).PSAS scores between January,June and December were surgery in the three groups(P <0.001).A further pairwise comparison between groups showed that the PSAS scores between groups A and B,A and C,B and C(P <0.001).At 6 months after surgery,the PSAS scores between group A and group C,Group B and Group C were statistically significant(P <0.001),and the PSAS scores between group A and Group B(P> 0.05).At 12 months,VSS scores between group A and C were statistically significant(P=0.002 <0.05),VSS scores between Group B and C(P=0.004 <0.05),and VSS scores between Group A and B(P> 0.05).Conclusions:To sum up,low power power electric knife skin joint improved micro inverted trapezoidal incision varus suture treatment can reduce thyroid surgery neck scar,make the incision more beautiful,and does not increase the operation time,hospital time,cost and complications,etc.,simple and easy to do,good safety,has good clinical application effect,worthy of clinical application. |
