Objective: The purpose of this paper is to conduct molecular typing of endometrial cancer based on the Proactive Molecular Classification Tool(Pro Mis E),to explore the guiding significance of Pro Mis E molecular typing for the treatment of endometrial cancer,and to evaluate the value of patient prognosis,so as to provide patients with personalized treatment plans.Method(s): The clinical data of endometrial cancer patients who underwent surgical treatment from February 2020 to October 2022 in the Department of Gynecology,Affiliated Hospital of Guilin Medical University,and underwent immunohistochemical(IHC)determination of p53 and mismatch repair(MMR)protein(MLH1,PMS2,MSH2,and MSH6)sequencing in paraffin sections were retrospectively collected.According to the results of MMR,p53 IHC,and POLE,the patients were divided into four groups according to Pro Mis E molecular typing: POLE mutation group,MMR-D group,p53 wt group,and p53 abn group.Compare the clinicopathological characteristics and prognosis of the four groups of patients,and analyze the overall situation and differences between groups from three aspects: clinical characteristics,pathological characteristics,and prognosis.Result(s): A total of 60 patients with endometrial cancer were collected in this study,including 4 in the POLE mutation group,13 in the MMR-D group,32 in the p53 wt group,and 11 in the p53 abn group.There were no significant differences in age,menopausal status,comorbidities,preoperative serum CA125,HE-4 levels,FIGO stage,depth of myometrial invasion,lymph node metastasis,lymphatic vessel space infiltration and progression-free survival among the four groups(P>0.05).There were significant statistical differences in pathological type,differentiation,ER and PR expression among the four groups of patients(P<0.05).The pathological grading of patients with POLE mutations was mainly low differentiation.The pathological grading of patients with POLE mutations was mainly low differentiation.The pathological grading of patients in the p53 wt group was mainly highly differentiated,and the pathological type was endometrioid carcinoma.All patients were positive for ER,and the positive expression rate of PR was higher than the negative rate.The OS of patients in the POLE mutation group,MMR-D group,p53 wt group,and p53 abn group were 100.0%,100.0%,96.9%,and 81.8%,respectively.There was a statistically significant difference between the p53 abn group and the POLE mutation group,MMR-D group,and p53 wt group(P<0.05).Conclusion(s): Pro Mis E molecular typing has a guiding role in predicting the prognosis of endometrial cancer patients.The prognosis of patients in the p53 abn group is worse than that in the POLE mutation group,MMR-D group,and p53 wt group.
