| Objective:To investigate the risk factors of endometrial cancer in asymptomatic postmenopausal women with endometrial thickening,and to explore the endometrial thickness value at which endometrial malignant lesions are more likely to occur in asymptomatic postmenopausal women with endometrial thickening.Methods:From January 2020 to June 2022,asymptomatic postmenopausal women in the First Affiliated Hospital of Hebei North University with endometrial thickening suggested by transvaginal color Doppler ultrasound and further underwent hysteroscopy and endometrial biopsy in our hospital were retrospectively analyzed.Their relevant data were collected and recorded.Endometrial thickness,age,age of menarche,age of menopause,duration of menopause,gravidity,parity,body mass index(BMI),pathological results of endometrial biopsy,and presence or absence of hypertension and diabetes were included in this study.The pathological results were divided into benign endometrial lesions and malignant endometrial lesions.Independent sample t test orχ~2test was used to analyze the general clinical data of the two types of patients.Binary Logistic regression was used to analyze the risk factors of endometrial malignant transformation,and ROC curve was used to predict the endometrial thickness value of asymptomatic postmenopausal women with endometrial thickening who were more likely to have endometrial malignant lesions.Results:The pathological findings in 57.6%of asymptomatic postmenopausal women with endometrial thickening were endometrial polyps.Endometrial thickness,parity and diabetes had significant influence on endometrial malignant transformation(P<0.05).Binary Logistic regression analysis showed that endometrial thickness and diabetes mellitus were independent risk factors for endometrial malignant lesions in asymptomatic postmenopausal women with endometrial thickening(P<0.05).ROC curve showed that when ET=11.5mm for asymptomatic postmenopausal women with endometrial thickening,the maximum area under the curve was 0.757,the sensitivity was 1.000,and the specificity was 0.470.Conclusions:Endometrial polyps are the most common pathological type in asymptomatic postmenopausal women with endometrial thickening.Endometrial thickness and diabetes mellitus are independent risk factors for malignant endometrial lesions in asymptomatic postmenopausal women with endometrial thickening.When ET≥11.5mm,asymptomatic postmenopausal women with endometrial thickening are more likely to find endometrial malignant lesions. |
