Objective(s): Retrospectively analyze the basic information,laboratory-related detection indicators and abdominal fat area of patients with different types of stones,analyze the correlation between each index and the type of urinary stones,and explore the related factors of various types of stone formation and stones combined with hyperuricemia Influencing factors of stone types in patients with chronic kidney disease,providing a basis for further prevention and treatment of urinary stones.Methods: In this study,surgical treatment(percutaneous nephrolithotripsy,ureteroscopic lithotripsy,flexible ureteroscopic lithotripsy,renal pelvis/ The basic data and laboratory-related detection indicators of patients with urinary calculi who underwent stone composition analysis were collected and analyzed retrospectively.Finally,165 cases were included,including 85 cases in the calcium oxalate stone group,30 cases in the uric acid stone group,and 50 cases in the infection stone group.Among the enrolled cases,there were 86 patients with stones complicated with hyperuricemia(HUA),including 42 cases in the calcium oxalate stones complicated with HUA group,21 cases in the uric acid stones complicated with HUA group,and23 cases in the infectious stones complicated with HUA group.Abdominal CT images at the umbilical plane were intercepted,and image J software was used to measure waist circumference(waist circumferencr,WC),total abdominal fat area(Total Fat Area,TFA),visceral fat area(Visceral Fat Area,VFA),subcutaneous fat area(Subcutaneous Fat Area,SFA)and other human body indicators.The basic information,laboratory indicators,fat area,etc.of the three types of stone patients were compared,and the indicators with differences were compared between the three groups.The two groups with statistical differences in the main observation indicators were further analyzed and multivariate logistic regression analysis was performed to explore the correlation between each clinical indicator and stone type.The basic information,laboratory indicators,and fat area of the three groups of patients with stones complicated with HUA were compared,and the indicators with statistically significant differences were analyzed by multi-category disordered Logistic regression to explore the influencing factors of stone types in patients with stones complicated with HUA.Results: The proportion of male patients in calcium oxalate stone group and uric acid stone group was higher than that of female patients,and the proportion of male and female patients in infection stone group was roughly equal.There were statistical differences in age,urine p H value,albumin,serum uric acid,serum creatinine,TFA,and VFA among the three groups of patients with urinary calculi(P<0.05).The age,blood uric acid,TFA and VFA of the uric acid stone group were higher than those of the other two groups,and the p H value was lower than that of the other two groups.The albumin level in the calcium oxalate stone group was higher than that in the other two groups(P<0.05).Further,the TFA,VFA,urine PH value,and blood uric acid of the three groups of stone patients were compared between the three groups.There was no statistical difference in TFA,VFA,and blood uric acid between the calcium oxalate stone group,the uric acid stone group,and the infection stone group(P>0.05).There were significant differences in age,serum creatinine,serum uric acid,urine p H,TFA,VFA,and SFA between the infectious stone group and the uric acid stone group(P<0.05).The univariate analysis(P<0.01)of the infected stone group and uric acid stone group was included in the multivariate logistic regression analysis.The results showed that the urine p H value [OR=0.151,95%CI(0.044-0.523),P =0.003] and TFA [OR=36.707,95%CI(3.747-359.632),P=0.002] were independent influencing factors of uric acid stone formation.Among the three groups of patients with stones complicated with HUA,there were statistically significant differences in age,albumin,serum creatinine,urine p H,BMI,and TFA(P<0.05).The age,serum creatinine,BMI,and TFA in the uric acid stone combined with HUA group were higher than those in the calcium oxalate stone combined with HUA group and the infected stone combined with HUA group.Further multi-categorical and disordered Logistic regression analysis of the factors that met the screening conditions(P<0.1)in univariate analysis showed that in patients with stones complicated with HUA,the calcium oxalate combined with HUA group was used as the control group,age [OR=0.911,95% CI(0.852-0.975),P=0.007] was an independent influencing factor for infectious stones;age [OR=1.123,95%CI(1.013-1.244),P=0.027],serum creatinine [OR=1.017,95%CI(1.003-1.030),P=0.013],urine p H value [OR=0.195,95%CI(0.048-0.788),P=0.022] are independent influencing factors of uric acid stones.Conclusion(s): 1.In uric acid stones and infection stones,TFA and urine p H value are independent influencing factors for the formation of uric acid stones;the higher the urine p H value,the higher the possibility of stones being infection stones.2.In patients with stones complicated with HUA,age,urine p H value,and serum creatinine are independent influencing factors for uric acid stones;age is an independent influencing factor for infectious stones.
