| Objective:By collecting the historical medical datas of patients who were diagnosed with Subacute Thyroiditis in Guangdong Provincial Hospital of Chinese Medicine,then to analyze the difference in the efficacy between two administrations(one with TCM and western medicine,another with western medicine only)and summarize the medication rule of Subacute Thyroiditis,which can provide some reference for clinical treatment of this disease.Methods:Medical datas of patients diagnosed with Subacute Thyroiditis in Guangdong Provincial Hospital of Chinese Medicine(including Tianhe and Xiatang outpatient departments)from January 2015 to December 2021 were collected by means of historical data analysis.According to different treatment regimens,they were divided into TCM group(oral TCM decoction or granule + hormone: Prednisone acetate tablets)and non-TCM group(oral hormone:Prednisone acetate tablets),laboratory indicators before and after treatment and the scores changes of TCM syndromes during treatment were recorded,and differences in laboratory indicators improvement,TCM syndrome scores improvement,disappearance time of thyroid pain and total course of treatment were compared between the two groups after treatment.SPSS 25.0 software was used for statistical analysis to analyze and explore the advantages of integrated Traditional Chinese and Western medicine in the treatment of subacute thyroiditis.Using statistical analysis software SPSS 25.0 to explore the advantages of integrated traditional Chinese and Western medicine in SAT,and the effective TCM prescriptions and corresponding syndrome types were recorded into the TCM inheritance auxiliary platform for statistics and mining.Results:The clinical symptoms of the two groups of patients included in this study were significantly improved after treatment,and the laboratory test indexes were improved or returned to normal level.However,the TCM group was superior to the non-TCM group in the whole course of treatment,pain disappearance,ESR recovery time,improvement of TCM syndrome score in the second week and overall curative effect.The median of the whole course of treatment in TCM group was 77.00(55.00,99.75).The median time of pain disappearance was 25.00(14.00,41.75),and the median time of ESR recovery was 60.00(40.25,77.00).In the second week,the improvement of TCM syndrome scores was markedly effective in 25 cases(23.1%),effective in 75 cases(69.4%),and ineffective in 8cases(7.4%).At the 12 th week of treatment,85 cases(78.8%)were under clinical control and 23 cases(21.3%)were significantly effective.The median of the total course of treatment in non-TCM group was 93.00(75.75,117.00).The median time of pain disappearance was 35.00(28.00,60.00),and the median time of ESR recovery was 80.00(60.00,90.00);In the second week,the improvement of traditional Chinese medicine syndrome score was markedly effective in 14 cases(12.1%)and effective in 93 cases(80.2%),Invalid in 9 cases(7.8%);At the 12 th week of treatment,71 cases(61.2%)were under clinical control,45 cases(38.8%)had remarkable effect.The disease syndrome type is the most frequent occurrence of heat-toxic syndrome,followed by phlegm-heat stasis syndrome and fire-stagnation-phlegm-resistance syndrome,then summarize the core prescriptions of the three certificate types.A total of 219 traditional Chinese medicines were used,of which the top 12 drugs were: Bupleurum,Forsythia,Scutellaria,Licorice,Poria,Silkworm,Pinellia,Zhe Fritillaria,Paeonia lactiflora,Cortex Moutan,Coix Seed,honeysuckle.The most commonly used traditional Chinese medicines are cold medicine,followed by warm and flat medicines.In terms of medicinal taste,bitter,sweet and acrid drugs were the most used,and astringent drugs were used the least.In terms of meridians belonging to medicines,most of them belong to the lung meridian,followed by those belonging to the liver meridian,stomach meridian and the spleen meridian.In terms of the efficacy of TCM,heat-clearing drugs are the most popular,followed by tonic drugs,and external-relieving drugs are the third.According to the analysis of prescription rules,a total of 5 new prescription combinations were obtained.Conclusion:In this study,the male to female ratio of SAT patients was about 1:3.77,and the onset age was mostly between 35 and 55 years old.Through statistical analysis,the TCM group had statistical significance in the relief of TCM syndromes in the second week,pain disappearance time,ESR recovery time and total course of treatment,indicating that the treatment effect of TCM combined with western medicine is better than that oral western medicine alone.In this study,SAT was mainly characterized by the syndrome of heat-toxic syndrome,phlegm-heat stasis syndrome and fire-stagnation-phlegm-resistance syndrome,which is consistent with the current view that exogenous、wind、heat and poison is the evil and the internal cause is emotional disorder.At the same time,the drugs mainly focus on clearing and purging the fire and heat,tonifying deficiency,relieving external appearance,soothing liver and regulating qi,resolving phlegm and removing stasis,promoting blood circulation and removing blood stasis,which are consistent with the syndrome types.The extracted core drug network of syndrome type and the potential prescriptions excavated can be used as basic prescriptions to guide the clinical medication of SAT. |
