ObjectiveTo describe the distribution of TCM syndrome types and motor subtypes of delirium patients,and to explore the relationship between different TCM syndrome types and motor subtypes of delirium patients and their short-term prognosis.MethodsA total of 152 delirium patients were admitted to the Intensive Care Unit(ICU),Emergency Intensive Care Unit(EICU),Neurological Intensive Care Unit(NICU)and neurosurgical intensive care unit of Foshan Hospital of traditional Chinese Medicine from February 2022 to February 2023.According to the standard,the patients were classified according to TCM syndrome differentiation and western medicine,and the length of mechanical ventilation,delirium,ICU hospital stay and other items were collected and recorded.The distribution of TCM syndrome types and motor subtypes of delirium patients were described by statistical method,and the relationship between different TCM syndrome types and motor subtypes delirium patients and their short-term prognosis was discussed.ResultsA total of 152 cases were included in this study,including 84 males(55.3%)and 68females(44.7%),with an average age of(62.42 ± 11.29)years.(1)In the motor subtypes,hyperactive type(42.8%)>mixed type(40.8%)>hypoactive type(16.4%),in which hyperactive type is the most and hypoactive type is the least.(2)In the distribution of TCM syndromes,the syndrome of heat disturbance(38.8%)> deficiency syndrome(34.8%)>qi and blood stasis(26.3%),among which the number of heat disturbance syndrome is the largest.(3)The mortality rates of each motor subtype were as follows: hyperactive type(16.9%),mixed type(37.1%)and hypoactive type(40.0%).The mortality of hyperactive type was the lowest,and there was significant difference between the other two types(P<0.05).There was no significant difference in mortality between the other two types(P>0.05).The total length of hospitalization in hyperactive type,mixed type and hypoactive type were 24.00(3.00,55.5),39.00(18.75,63.25)and 27.00(13.00,48.50)days,respectively.The total hospital stay in hyperactive type was significantly lower than that in hypoactive type(P<0.05).There was no significant difference in total length of stay among other types(P>0.05).The hospitalization time of ICU in hyperactive type,mixed type and hypoactive type was 7.00(3.00,15.5),11.00(3.00,20.00)and 16.00(8.50,28.00),respectively.The length of ICU hospitalization in hyperactive type was significantly lower than that in hypoactive type,but there was no significant difference in ICU hospitalization duration among other types.The duration of delirium in the hyperactive group,the mixed group and the hypoactive group was 3.00(1.00,6.00)days,2.50(1.00,4.25)days and 6.00(4.00,9.00)days respectively,and there was significant difference between the hypoactive group and the other two types(P<0.05).There was no significant difference in the duration of delirium between the other two types(P>0.05).(4)The mortality rates of various TCM syndrome types were as follows: deficiency syndrome(65.9%)> heat disturbance syndrome(22.7%)> qi stagnation and blood stasis syndrome(11.4%).The mortality rate of deficiency syndrome was the highest,and there was significant difference between the other two TCM syndrome types(P < 0.05).There was no significant difference in mortality between the other two syndrome types(P > 0.05).There was no significant difference in delirium duration,ICU hospital stay and total hospital stay among different TCM syndrome types(P > 0.05).ConclusionIn this study,there are the most patients with hyperactive delirium in motor subtypes and the most heat disturbance syndrome in TCM syndrome types.In motor subtypes,the short-term prognosis of delirium patients in hypoactive group and mixed group was worse than that in hyperactivity group,and the short-term prognosis of delirium patients with deficiency syndrome in TCM syndromes was the worst.
