| Objectives:1.To learn about the nursing demands of pre-hospitalized patients before official admission,discuss about the influence factors of nursing demands of pre-hospitalized patients before admission,and lay a foundation for the making of the nursing management plan of pre-hospitalized patients before admission in the later stage.2.To make a nursing management plan for pre-hospitalized patients before admission.3.To verify its effect and rationality through the application of the nursing management plan for pre-hospitalized patients before admission and provide a reference basis for the standardized management of pre-hospitalized patients.Methods:1.Survey on the current conditions of nursing demands of pre-hospitalized patients before admissionThrough literature research,combined with the related research results of admission preparation of patients,the related requirements and contents of admission preparation of patients were determined.After group discussion,the questionnaire of nursing demands of pre-hospitalized patients before admission was developed.200pre-hospitalized patients were selected with the convenience sampling method for a questionnaire survey.Through the questionnaire survey,we had a good understanding of the nursing demands of pre-hospitalized patients before admission and analyzed their influence factors.2.Making of nursing management plan for pre-hospitalized patients before admissionThrough literature research,combined with the survey results of the current conditions of nursing demands of pre-hospitalized patients before admission,the first draft of the nursing management plan for pre-hospitalized patients before admission was developed.It was revised and improved through the demonstration of 10 experts.Finally,the nursing management plan for pre-hospitalized patients before admission was determined.3.Application of the nursing management plan for pre-hospitalized patients before admission100 pre-hospitalized patients in a first-class Grade 3 general hospital in Yunnan Province from February 2022 to September 2022 were selected randomly from in the Departments of Cardiology,Oral and Maxillofacial Plastic Surgery,and Hepatological Surgery each as the research objects.A control group and a test group were set for each of the three departments.They were numbered according to the admission order of pre-hospitalized patients who meet the inclusion and exclusion criteria in each department.The 50 patients who handled the pre-hospitalization procedures first were considered as the control group and the 50 patients who handled the pre-hospitalization procedures later were considered as the test group.The routine pre-hospitalization nursing management before admission was implemented for the control group,while the test group was managed according to the nursing management plan for pre-hospitalized patients before admission.SPSS 25.0 was used for statistical analysis of the waiting time of admission,hospital stay,and hospitalization expenses of the two groups of patients.The admission preparation score and hospitalized patient satisfaction of two groups of patients were compared,and the feasibility and effectiveness of the nursing management plan for pre-hospitalized patients before admission were discussed.Results:1.Survey results of the current conditions of nursing demands of pre-hospitalized patients before admission:(1)The score of nursing demands of pre-hospitalized patients before admission was(3.71±0.46)points.Compared with the assignment of the Likert 5-level score of the self-developed questionnaire for nursing demands of pre-hospitalized patients before admission,the score of 3.1-4 points was relatively high,so the score of nursing demands before admission was relatively high.(2)The scores of the following dimensions of nursing demands of pre-hospitalized patients before admission were from high to low: psychological support,disease knowledge guidance,project guidance to be completed in pre-hospitalization,living arrangement for hospitalization and environmental guidance,and follow-up demand.(3)Results of univariate analysis: There were differences in the scores of nursing demands of pre-hospitalized patients in the aspects of age,educational level,number of times of hospitalization,whether there was a companion,and main caregiver.Results of the multiple regression analysis were as follows.The influence factors of nursing demands of pre-hospitalized patients before admission included educational level,number of times of hospitalization,whether there was a companion,and main caregiver.The fitting equation is as follows.Nursing demands of pre-hospitalized patients before admission=3.135+0.69*educational level+0.196*whether there is a companion-0.43*number of times of hospitalization+0.025*main caregiver.If the square of R is 0.354,it indicates that the dependent variable of the multiple linear regression equation(score of nursing demands of pre-hospitalized patients before admission)can be explained by independent variables(age,educational level,number of times of hospitalization,whether there is a companion,and main caregiver).2.Making of the nursing management plan for pre-hospitalized patients before admission(1)A total of 10 experts in the fields of hospital management,nursing management,clinical medicine,and clinical nursing were invited for expert argumentation on the nursing management plan for pre-hospitalized patients before admission.The age of experts was 37-58 and the years of working were from 15 to 40 years.9 experts had senior professional titles and 1 expert had an intermediate professional title.The experts were from three first-class Grade 3 hospitals.(2)Expert argumentation results: The lowest score given by experts for the plan was(4.20±1.03)points and the highest score was(4.90±0.32)points.The lowest score given by experts for the process was(4.70±0.48)points and the highest score was 5 points.According to the opinions of experts,contents of safety management and multidisciplinary collaborative management,and optimization processes were added to the nursing management plan for pre-hospitalized patients before admission.Finally,the nursing management plan for pre-hospitalized patients before admission was determined.3.Application effect of nursing management plan for pre-hospitalized patients before admission(1)Comparison of baseline information: There was no statistically significant difference in the scores of general information between two groups of pre-hospitalized patients(P>0.05).They were comparable.(2)Comparison of results of waiting time,hospital stay,and hospitalization expenses of patients: The waiting time of admission and hospital stay of pre-hospitalized patients in the Departments of Cardiology and Oral and Maxillofacial Plastic Surgery in the test group were shorter than those of patients in the control group,and hospitalization expenses were lower than that of patients in the control group.The difference was statistically significant(P<0.05).The waiting time of admission of pre-hospitalized patients in the Department of Hematological Surgery in the test group was shorter than that of patients in the control group.The difference was statistically significant(P<0.05).There was no statistically significant difference in the hospital stay and hospitalization expenses of the two groups in the Department of Hepatological Surgery(P>0.05),but the hospital stay of patients in the test group was shorter than that of patients in the control group and hospitalization expenses were lower than that of patients in the control group.(3)Comparison of admission preparation scores: The admission preparation evaluation scores of the test group in the Departments of Cardiology,Oral and Maxillofacial Plastic Surgery,and Hepatological Surgery were higher than those of the control group.The difference was statistically significant(P<0.05).(4)Comparison of satisfaction results of hospitalized patients: The scores of the dimensions of hospitalization satisfaction and overall satisfaction score of pre-hospitalized patients in the Department of Cardiology in the test group were higher than those of patients in the control group.The difference was statistically significant(P<0.05).The scores of the dimensions of hospitalization satisfaction and overall satisfaction score of pre-hospitalized patients in the Department of Oral and Maxillofacial Plastic Surgery in the test group were higher than those of patients in the control group.The difference was statistically significant(P<0.05).The scores of pain and medication management,convenience and continuity,and demand satisfaction,and overall satisfaction core of pre-hospitalized patients in the Department of Hepatological Surgery in the test group were higher than those of patients in the control group.The difference was statistically significant(P<0.05).There was no statistically significant difference in the scores of nurse communication,doctor communication,and hospital environment between the two groups(P>0.05),but the scores of the test group were higher than those of patients in the control group.Conclusions:1.The nursing demands of pre-hospitalized patients before admission are at a high level.Nursing management of pre-hospitalized patients before admission is necessary.The influence factors include educational level,number of times of hospitalization,whether there is a companion,and main caregiver.The nursing demands of pre-hospitalized patients before admission are positively correlated with educational level,whether there is a companion,and main caregiver,and negatively correlated with the number of times of hospitalization.2.Compared with conventional nursing management,the nursing management plan for pre-hospitalized patients before admission made in this study can help to decrease the waiting time of admission,hospital stay,and hospitalization expenses,increase the admission preparation score,and improve the satisfaction of hospitalized patients.It indicates that the nursing management plan for pre-hospitalized patients before admission is effective for improving hospital operation efficiency,admission preparation,and patient satisfaction.This study provides a theoretical basis for the admission service management of pre-hospitalized patients and a reference for the improvement of standardized management of pre-hospitalized patients. |
