Research On The Elder Care Services Of Medical And Nursing Integration Based On Personal Care Plan

According to the "Seventh National Population Census Bulletin," Chinese aging is worsening,owing to an increase in the old population,a high frequency of chronic illnesses,and poor physical health.The aging society aggravates the pension burden of the elderly and their families,and reduces the elderly living standards and happiness index in old age.In order to promote the development of Chinese pension cause,and to implement General Secretary Xi Jinping’s concept of "Transforming the focus on treating diseases to centering on people’s health,paying attention to the whole life cycle and the whole process of health",the integration of medical care and elderly care based on individual care plan model which is the only way for the development of the pension industry.The medical and elderly care integration service model aims to integrate medical and elderly care resources,and realize the interconnection of medical and elderly care data and health management.The personal care plan model is a further embodiment of the integrated medical and nursing service model.The personal care plan services include long-term care services,medical care services,rehabilitation services,balanced diet services,psychological care services.The difficulty in its implementation lies in the lack of refined management of service content,the supply capacity is far from meeting the multi-level service needs of the elderly,and the lack of data interaction in some medical care services.Therefore,this paper realizes the standardization of service content based on the theory of personal care plan,solves the problem of insufficient supply capacity by constructing an optimal allocation model of personnel,and expands the functions of the existing pension service management system to realize the effective combination of medical and nursing information.The specific research contents are as follows:Firstly,through the statistical analysis of the care measures data in the personal care plan model,it is concluded that there are problems such as the lack of refined division of service measures and the insufficient standardization of service content texts in the formulation and classification of service content in the elderly home in G City.To this end,this paper standardizes the content of specific measures to achieve refined service management.Secondly,based on the standardized historical data of care measures,the time series ARIMA forecast model is used to forecast the demand for various services in 2022.Furthermore,in order to alleviate the imbalance between supply and demand,a mathematical model for the optimal allocation of service personnel was established with the optimization goal of minimizing the cost of human resources.LINGO solver was used to obtain the optimal allocation plan for each month.Finally,in order to realize the combination of medical and elderly care data and provide a one-stop medical and elderly care service environment,this paper realizes the refinement and standardization of the personal care service model in the existing nursing home management system,and expands the demand forecasting module,The service personnel allocation module and the old-age sign warning module promote the effective application of the personal care plan and the medical-nursing integration model,further guaranteeing and improving the old-age experience and living standards of the elderly.
