Objective Lasso regression was used to screen the indexes related to UA in HUA patients,and the model was established to analyze the risk of HUA.Based on the risk results of HUA,the relationship between HUA and liver,kidney function and lipid metabolism was explored to provide a theoretical basis for rational prevention and treatment of HUA.Methods 1.Population:7703 medical examinees in Ningxia were collected retrospectively.Examinees were divided into HUA group and non-HUA group.The differences of age,sex,liver and kidney function,blood lipid and blood glucose between the two groups were compared.The influencing factors of HUA were screened by Lasso regression and included in multiple linear regression and Logistic regression.2.Forty6-week-old male SPF grade SD rats were randomly divided into experimental group(5%fructose+100mg/kg/d oxazine potassium,n=20)and control group(n=20).After 4 weeks of feeding,the general condition and laboratory indexes between the two groups were compared.The indexes of liver and kidney function,blood lipid and blood glucose of experimental group at 4 weeks were screened by Lasso regression and included in multiple linear regression.At the end of the 4th week of the experiment,the experimental group stopped 5%fructose intervention,and was randomly divided into potassium oxazine group(100mg/kg/d potassium oxazine,n=10)and no potassium oxazine group(n=10).The general condition and laboratory indexes of potassium oxazine group,no potassium oxazine group and control group was compared at 4th,6th and 8th week.Results 1.Population:Of the 7703 people examined in Ningxia,1550 were diagnosed with HUA,the prevalence rate was 20.12%,the average age of male examined was 54.42years,the prevalence rate was 26.4%,and the average age of female examined was 50.36years,the prevalence rate was 3.5%.Liver function(ALT,AST),renal function(BUN,Scr),blood lipid(TC,TG,HDL-C,LDL-C),FPG and UA in patients with HUA were statistically significant compared with those without HUA(P<0.001).Lasso regression compresses the coefficients of ALT,AST,BUN,Scr,TC,TG,HDL-C,LDL-C,and FPG to determine the optimal lambda=0.901.The selected variables are ALT,AST,Scr,and TG.Taking ALT,AST,Scr and TG as independent variables and UA as dependent variables,the multivariate linearregressionequationwasestablishedasfollows:YUA=404.263+0.138XALT+0.293XAST+0.600XScr+2.374XTG.The results of logistic regression showed that the incidence of HUA increased with the add of ALT(OR=1.015,95%CI:1.012~1.019),Scr(OR=1.054,95%CI:1.049~1.059)and TG(OR=1.311,95%CI:1.263~1.361),AUC and 95%CI of the ROC were 0.771(0.759~0.783).2.Animal experiment:the feed intake of experimental group(21.47±0.90)was higher than that of control group(23.53±1.35)at the 4th week;the fructose water intake of experimental group(126.29±27.56)was higher than that of control group(45.96±5.12);the UA of experimental group(107.25±22.47)was higher than that of control group(91.31±19.87);the ALT of experimental group(52.15±14.93)was higher than that of control group(43.35±4.33);the AST of experimental group(175.89±32.91)was higher than that of control group(155.05±25.68);the Scr of experimental group(29.68±3.85)was higher than that of control group(26.20±3.28),with P<0.05.Lasso regression determined lambda=98.556,the selected variables include:ALT,BUN and Scr,the equation into the YUA=-383.717+0.676XALT+31.551XBUN+10.589XScr,adjust R2=0.825.At week 6,the feed consumption of potassium oxazine group(24.69±1.55)was higher than control group(22.97±1.75),and the BUN of potassium oxazine group(8.97±0.90)was higher than control group(7.87±1.01)and no potassium oxazine group(7.83±0.51),P<0.05.At the 8th week,the drinking water consumption of no potassium oxazine group(50.28±4.64)was higher than control group(44.93±5.33)(P<0.05).In addition,there were no significant differences in liver function,renal function,lipid metabolism and other parameters between the 6 and 8weeks control,potassium oxazinate and no potassium oxazinate(P>0.05).Conclusion 1.The prevalence rate of HUA in Ningxia is higher than the national average,and that is higher in male than in female.HUA is related to ALT,AST,BUN,Scr,TC,TG,HDL-C,LDL-C and FPG.The level of UA increased with the level of AST,Scr and TG.2.Elevated ALT,Scr and TG suggest an increased risk of HUA.3.Fructose increases the risk of HUA,elevates UA levels and increases the burden on the liver and kidneys.4.Liver and kidney injury and UA levels can be recovered to normal within a short time after stopping fructose intake.
