| Objective By analyzing the levels of serum endotoxin and IL-6 in AIDS patients complicated with bacterial infection,the predictive diagnostic value of endotoxin and IL-6 in AIDS patients complicated with bacterial bloodstream infection was discussed,so as to find reliable indicators for early diagnosis of AIDS patients complicated with bloodstream infection.Methods From August 2008 to August 2021,a total of 81 patients with AIDS complicated with bacterial infection were collected in our hospital.According to whether the blood culture of the patients was positive or not,the patients were divided into 35 cases in the bloodstream infection group and 46 cases in the local infection group,including 35 cases in the bloodstream infection group and 46 cases in the local infection group.Collect the general data(gender,age,route of AIDS transmission)of all selected patients and related laboratory tests,including CD4+T lymphocyte count,microbial culture results,laboratory indicators such as endotoxin,IL-6,white blood cell count(WBC),lymphocyte count,CRP,neutrophil count,etc.,and compare the differences between the two groups in general data and the bacterial spectrum of patients infected.T-test was used to analyze the difference of perfect serum endotoxin and IL-6 levels in the first 24 hours of blood culture between the two groups when they were suspected of blood infection,so as to explore their early predictive value for AIDS complicated with bacterial bloodstream infection,and ROC receiver operating characteristic curve curve was drawn to compare their individual diagnostic value and joint predictive diagnostic value.Results 1.The levels of endotoxin and IL-6 in bloodstream infection group were higher than those in local infection group,and the difference was statistically significant(P < 0.05).The levels of endotoxin in bloodstream infection group and local infection group were(0.09±0.03)EU /ml and(0.05±0.15)EU /ml respectively,and the levels of IL-6 were(37.66±26.32)pg/ml and(24.64±13.84)pg/ml respectively.2.In the bloodstream infection group,the endotoxin levels in the gram-negative bacteria group and the gram-positive bacteria group were(0.11±0.03)EU/ml and(0.07±0.02)EU/ml/,respectively,and the difference was statistically significant(P <0.05).The levels of IL-6 in Gram-negative bacteria group and Gram-positive bacteria group were(44.27±27.28)pg/ml and(31.42±24.50)pg/ml,respectively,and the difference was not statistically significant(P > 0.05).3.Compared with the local infection group,the area under the ROC curve(AUC)of endotoxin was 0.88,P < 0.01,the sensitivity at the critical value was 82.9%,and the specificity was 84.8%.The AUC of serum IL-6 was 0.64,and the sensitivity and specificity at the critical value of P < 0.05 were 40.0% and 95.7%.AUC(endotoxin)>AUC(IL-6),P < 0.05.The area under ROC curve(AUC)was 91.4%,P < 0.01.Conclusion 1.The levels of endotoxin and IL-6 are helpful to predict and diagnose AIDS patients complicated with bacterial bloodstream infection.2.Endotoxin level has a certain predictive diagnostic value for AIDS complicated with bacterial bloodstream infection caused by G-bacteria.3.The value of endotoxin in early prediction and diagnosis of AIDS complicated with bacterial bloodstream infection is better than IL-6,and the combined evaluation of endotoxin and IL-6 is more helpful for early diagnosis of AIDS complicated with bacterial bloodstream infection. |
