Purpose:By observing the qualitative and objective indicators of tongue image in inpatients diagnosed with angina pectoris with syndrome of phlegm and blood stasis +syndrome of heat retention blood vessel and simple syndrome of phlegm and blood stasis and unstable angina pectoris,to explore the difference of tongue image characteristics between the two syndromes,and to explore the pathogenesis of the response of tongue image.Material and method:A total of 177 inpatients were selected from the department of Cardiology,Affiliated Hospital of Liaoning University of Traditional Chinese Medicine,who were diagnosed by TRADITIONAL Chinese medicine as patients with unstable angina pectoris with the combination of syndrome of phlegm and blood stasis + syndrome of heat retention blood vessel and the combination of simple syndrome of phlegm and blood stasis,according to inclusion and exclusion criteria.Tongue images were taken in the morning of the next day and the morning of discharge in all patients.Tongue image indicators include:tongue color,tongue shape,moss color,moss.Adobe Photoshop 2020 color extraction tool was used to extract the RGB and HSV color space values of tongue texture and tongue coating area in 5 partitions respectively.The database was imported into SPSS26.0 data statistical software,and independent sample T-test was used for counting data conforming to normal distribution,while non-parametric rank-sum test was used for counting data not conforming to normal distribution.Rank sum test is used for measurement data with grade characteristics.Chi-square test is used for measurement data without grade characteristics that require different statistical composition ratios.Results:1.The group of unstable angina pectoris with syndrome of phlegm and blood stasis +syndrome of heat retention blood vessel has higher characteristics of red tongue image,which reflects the higher proportion of hot syndrome element in this syndrome type.There was statistical difference in the percentage of tongue prick in the syndrome of phlegm and blood stasis + syndrome of heat retention blood vessel group compared with syndrome of phlegm and blood stasis.There was statistical difference in the characteristics of cracked tongue between syndrome of phlegm and blood stasis+syndrome of heat retention blood vessel group and syndrome of phlegm and blood stasis.There was no significant difference in the characteristics of fat and thin,old and young,and tooth marks.2.Compared with the group of phlegm and blood stasis,the color of tongue of syndrome of phlegm and blood stasis + syndrome of heat retention blood vessel group coating was statistically different,the color was more yellow.In unstable angina pectoris,there was a statistical difference in the degree of dryness and moss,while there was no significant difference in the amount of thick and greasy moss.3.Objectified tongue feature data showed that R value of region 1 was statistically different between the two groups.The results showed that the tongue color was more red in zone 1 of syndrome of phlegm and blood stasis + syndrome of heat retention blood vessel group.The other G,B,R+B/R+G+B,H,S and V groups had no statistical significance.The G value,R+B/R+G+B value,S value,V value and R/R+G+B value were statistically different between the two groups.Other R,B and H values were not statistically significant between the two groups.It indicates that the tongue color in zone 2 of the syndrome of phlegm and blood stasis+ syndrome of heat retention blood vessel group is more red,purple,darker and brighter than that in the phlegm-stasis interknot group.The values of R+B/R+G+B,R/R+G+B and S were statistically different between the two groups.Other R,G,H,B and V values were not statistically significant between the two groups.The results showed that the tongue color in zone 3 was more red,purple and darker in the group of syndrome of phlegm and blood stasis+ syndrome of heat retention blood vessel than in the group of syndrome of phlegm and blood stasis.The R+B/R+G+B values,S values and R/R+G+B values were statistically different between the two groups.Other R,G,H,B and V values were not statistically significant between the two groups.The results showed that the tongue color of area 4 was more red,purple and darker in the group of syndrome of phlegm and blood stasis + syndrome of heat retention blood vessel than in the group of syndrome of phlegm and blood stasis.The R+B/R+G+B and H values were statistically different between the two groups.Other R,G,S,B and V values were not statistically significant between the two groups.It indicates that the color difference of area 5 in the group of syndrome of phlegm and blood stasis + syndrome of heat retention blood vessel is larger and more purple than that in the group of phlegm and blood stasis.There were significant differences in R value,B value,R+G/R+G+B value,H value,S value and V value between the two groups,while there was no significant statistical difference in G value.It indicates that the color difference of tongue coating in area 1 in the syndrome of phlegm and blood stasis + syndrome of heat retention blood vessel group is larger than that in the phlegm and blood stasis group,and the color is more yellow,darker and brighter.There were significant differences in R value,B value,R+G/R+G+B value,H value,S value and V value between the two groups,while there was no significant statistical difference in G value.It indicates that the color difference of tongue coating in area 1 in the syndrome of phlegm and blood stasis + syndrome of heat retention blood vessel group is larger than that in the syndrome of phlegm and blood stasis group,and the color is more yellow,darker and brighter.There were significant differences in G value,B value,R+G/R+G+B value,H value and S value between the two groups,while there were no significant differences in R and V value.It indicates that the color difference of tongue coating in area 2 in the syndrome of phlegm and blood stasis+syndrome of heat retention blood vessel group is larger than that in the syndrome of phlegm and blood stasis group,and the color is more yellow and darker.There were significant differences in B value,G value,R+G/R+G+B value,H value and S value between the two groups,while there were no significant differences in R value and V value.Compared with the syndrome of phlegm and blood stasis group,the color of tongue coating in area 3 was more yellow and darker in the syndrome of phlegm and blood stasis + syndrome of heat retention blood vessel group.There were significant differences in B value,G value,R+G/R+G+B value,H value and S value between the two groups,while there were no significant differences in R and V value.The results showed that the color difference of tongue coating in area 4 was more yellow and darker in the group of syndrome of phlegm and blood stasis+syndrome of heat retention blood vessel than in the group of syndrome of phlegm and blood stasist.There were significant differences in G value,B value,R+G/R+G+B value,H value and S value between the two groups,while there were no significant differences in R and V value.The results showed that the tongue coating in the syndrome of phlegm and blood stasis + syndrome of heat retention blood vessel was more yellow and darker than that in the syndrome of phlegm and blood stasis group.Conclusion:1.The difference of tongue image features between unstable angina pectoris with the syndrome of phlegm and blood stasis+syndrome of heat retention blood vessel and simple syndrome of phlegm and blood stasis1.1 Differences of macroscopic tongue imagesCompared with the simple simple syndrome of phlegm and blood stasis,the syndrome of phlegm and blood stasis+syndrome of heat retention blood vessel shows that the tongue is more red and purple like the tongue,with more prickles and cracks in the tongue,and the moss is more yellow and dry.1.2 Objectify the difference of tongue color and moss colorCompared with the simple syndrome of phlegm and blood stasis,the tongue color in zone 1,zone 2,3 and 4 was more red,deeper and purple,and zone 5 was more purple in the syndrome of phlegm and blood stasis + syndrome of heat retention blood vessel group.Zone 1is brighter,darker and more yellow,while zone 2,3,4 and 5 are more yellow and deeper in the syndrome of phlegm and blood stasis+syndrome of heat retention blood vessel.1.3 Pathogenesis summary based on the difference of tongue images between the two groups In unstable angina pectoris,the syndrome of phlegm and blood stasis+syndrome of heat retention blood vessel group is based on spleen deficiency,phlegm-stasis heat as the standard,heat and disease evil as the main factor of current standard.Compared with the syndrome of phlegm and blood stasis,blood stasis is more severe.Qi heat and ying blood heat both exist,involving the viscera including heart,liver and gallbladder,spleen and stomach,etc.Although ying blood heat is abundant,ying Yin consumption is not injured,and jin injury is relatively more obvious.2.Methodological conclusionsThe Color numeric value of RGB and HSV can reflect the difference of tongue image features in different groups,and are more sensitive to the color difference and purple difference in different regions,which has practical value.
