| Purpose:To evaluate the efficacy and safety of Chaihu Longguoyasu Decoction by observing its prevention and treatment effect on cardiotoxicity caused by chemotherapy and targeted therapy of malignant tumors,and to discuss the mechanism of chaihu Longguoyasu Decoction.Materials and Methods:A total of 85 patients were collected in this study according to the criteria of intake and discharge,and 82 patients were finally completed.According to whether the patients voluntarily took TCM decoction,they were divided into the control group(42 cases)and the observation group(40 cases)(3 cases were excluded due to the interruption of TCM treatment).Control group: Chemotherapy alone or targeted therapy was adopted.According to the disease type,pathological type and patient’s own situation,chemotherapy and targeted therapy drugs were selected,the specific drug dose was given by calculating the patient’s body surface area and body weight,and preventive treatments such as nausea,liver protection,stomach protection and anti-allergy were given accordingly.The main chemotherapy drugs were paclitaxel,docetaxel,albumin-bound paclitaxel,capecitabine,gio,epirubicin and doxirubicin.The targeted drugs are pertuzumab and trastuzumab.There were 4 cycles of treatment in 21 days.Observation group: on the basis of the control group,the clinical symptoms were palpitation plus cinnamomum twigi,chest tightness/pain plus Fructus aurantii,Xiangfu,fructus trichosanthis,allium macrostemon,shortness of breath plus almond,Perilla fruit.Accompanied by symptoms of abdominal distention and fructus aurantii,tangerine peel,diarrhea and Yam,constipation and trichosanthis,radix scrophulariae,nausea and vomiting and bamboo shau,tangerine peel,difference and coke three fairy,chicken gold.On the first day of chemotherapy or targeted therapy,Chinese traditional medicine decoction should be taken orally for at least 7 days per cycle,with 21 days as one cycle,for a total of 4cycles.Changes of ecg before and after treatment,changes of cardiac color doppler ultrasound and myocardial enzyme spectrum related indexes before treatment and after 2 and 4 cycles of treatment,as well as TCM symptoms related to cardiotoxicity were observed.Changes of blood routine,liver and kidney function indexes,and tumor marker(CEA)were detected.Conclusions were drawn by comparative analysis of data between the two groups.Results:1.Ecg: there was no statistically significant difference in ECG between 2 groups before and after treatment(P>0.05).2.Myocardial enzyme profile: after 2 and 4 cycles of treatment,the abnormal number of CK-MB in both groups increased.Comparison between the two groups showed that there was no significant difference after 2 cycles of treatment(P>0.05),but significant difference after 4cycles of treatment(P<0.05);There was no significant difference in abnormal array of LDH and CK between the two groups(P>0.05).3.Color doppler echocardiography: There was no significant difference in LVEF value between the two groups after 2 cycles of treatment(P>0.05),but significant difference was found after 4 cycles of treatment(P<0.05);There was no significant difference in LVEDD value between 2 groups(P>0.05).4.CEA: CEA values in both groups were decreased before and after treatment,and the observation group was more significant than the control group,with statistically significant differences(P<0.05).5.Symptom improvement: in the improvement of palpitations and chest tightness/pain,the observation group was better than the control group after 4 cycles of treatment,and the difference was statistically significant(P<0.05);There was no significant difference in the improvement of shortness of breath between the two groups(P>0.05).6.Efficacy indicators: the effective rate of TCM symptoms in the two groups was 85% in the observation group and 67.5% in the control group.There was significant difference in efficacy between the two groups(P<0.05).7.Safety analysis: The blood routine and liver and kidney functions of patients in the two groups were observed,and it was found that white blood cells and neutrophils decreased after treatment compared with before treatment.However,it was more obvious in the control group,and the difference was statistically significant(P<0.05),while there was no statistical difference in the observation group(P>0.05).There was no statistical significance in the effect of treatment on liver and kidney function between the two groups(P>0.05).Conclusion:radix bupleuri keel oyster soup can improve left ventricular ejection fraction,reduction of abnormal CK-MB number is lower than the simplistic target therapy,and can alleviate toxicity caused by heart palpitations,chest tightness,chest pain and other symptoms,can make the tumor markers CEA is relatively simplistic target therapy reduced more significantly,no obvious bone marrow inhibition on security and kidney toxicity.Comprehensive analysis,the application of Bupleurum longbone oyster decoction can improve the cardiotoxicity caused by target drugs,achieve the therapeutic purpose,and can be further clinical study. |
