Evaluation Of He's Santong Method Intervention Based On TCM Constitution Identification In Myopic Children And Adolescent

Part 1 Constitutional correlation analysis and risk factors between myopia and traditional Chinese medicine in children and adolescentsObjective:To investigate the TCM Constitution and myopia related observational indicators in children and adolescents,and explore the correlation between the two groups.Methods:This trial took a cross-sectional study.347 children and adolescents aged 6-16 years,including 281 myopic and 66 non myopic patients,who visited the eye clinic of Beijing Traditional Chinese medicine hospital,Capital Medical University between July 2022 and September 2022 were included according to the proposed criteria.Indicators of general condition(name,gender,age,ethnicity,contact details,height,weight,dietary habits,time spent outdoors,sleeping time,and allergies),ophthalmic examination(naked eye vision,intraocular pressure,anterior segment condition,spherical equivalent,ocular axis),and type of TCM Constitution were collected for analysis.Results:①There was no significant difference in the distribution of nonmyopic and myopic groups at each age(P>0.05),showed a trend toward higher prevalence of myopia with advancing age.②The myopic group was taller than the non myopic group,and the difference was statistically significant(P≤0.05).③The non myopic group had more unbiased liking of diet(46.97%),followed by liking sweet food(30.30%)and liking light diet(10.61%),while the myopic group had more predominant liking sweet food(64.77%),followed by unbiased liking(14.59%)and liking spicy(7.12%),with significant differences(P≤0.05).④The time spent outdoors and sleeping time in the myopic group were significantly shorter than those in the nonmyopic group,and the difference was statistically significant(P≤0.05).⑤The proportion of subjects with allergies was higher in the myopic group than in the non myopic group(P>0.05).⑥The percentage of both parents of the subjects with myopia was as high as 59.43%(P≤0.05).⑦The ocular axis was significantly longer in the myopic group than in the nonmyopic group(P≤0.05),height had a positive linear correlation with the ocular axis(P≤0.05),and the degree of spherical equivalent had a negative linear correlation with the ocular axis(P≤0.05).⑧There was no significant difference in the TCM constitution ratio between the non myopic and myopic groups(P>0.05),and the TCM constitution types of the non myopic group were mainly plain and qualitative(24.2%),phlegm and wet(13.6%)and Qi dampness(12.1%),while that of the myopic group was less plain and qualitative(13.9%)than that of the non myopic group,and the biased constitution was mainly Qi dampness(23.1%),special(14.6%)and phlegm and wet(11.4%).⑨As the number of equivalent spherical mirror degrees increased,the composition ratio of flat and mass decreased,the composition ratio of biased body Rose(P≤0.05),and the composition ratio of qi deficiency and qi depression showed an increasing trend(P>0.05).⑩As the axial length of the eye increased,there was a decreasing trend in the constitu-tion ratio of flat and mass,a rising trend in the constitu-tion ratio of biased body(P>0.05),and an increasing trend in the consti-tution ratio of bipolar disorder(P>0.05).?The differences were not statistically significant(P>0.05),as the following diets:qiyushenxi spicy(41.7%),pinghue unbiased liking(29.2%),teshenshenshenxi salty(27.3%),qixushenshenshenxi acid(25.0%),and phlegm wet liking sweet(19.2%).Conclusion:①Genetic,height,axial length,behavior,environment,and allergic diseases may be risk factors for the development of myopia.②There was a correlation between TCM Constitution and myopia,and most dampness was in the biased constitution of myopic subjects,followed by idiopathic and phlegm dampness.③The influencing factors of myopia and constitution are consistent,or prevention and control of myopia can be assisted by conditioning constitution.④There was a statistically significant difference in the number of equivalent spherical degrees between different TCM Constitution in myopic subjects.Part 2 Short term clinical trial of He’s acupuncture-moxibustion three removing obstruction method for low-grade simple myopia in adolescents with bipolar disorderObjective:Evaluation of the scientific norm the short-term clinical efficacy of the He’s acupuncture-moxibustion three removing obstruction method for the intervention of mild simple myopia in adolescents with bipolar disorder.Methods:This trial took a prospective randomized controlled trial.In accordance with the proposed inclusion and exclusion criteria,depressed adolescents with mild simple myopia who presented to the ophthalmology outpatient clinic of Beijing Traditional Chinese medicine hospital,Capital Medical University between July 2022 and April 2023 were divided into treatment and control groups according to a random number table and the order of their visits.A total of 26 patients(52 eyes)in the treatment group and 26 patients(52 eyes)in the control group were treated with optical correction alone and the study lasted for 12 weeks.The visual acuity of the naked eye,the degree of spherical equivalent,axial length,corneal curvature before and after treatment,as well as safety indicators and adverse effects were evaluated in both groups.Results:①The baseline data,such as gender,age,naked eye visual acuity,spherical equivalent,axial length,and keratometry,were not significantly different between the treatment and control groups before treatment(P>0.05).②After treatment,the treatment group showed significant improvement(P≤0.05)in the depression score compared with the previous period,no significant progression(P>0.05)in the visual acuity and keratometry of the naked eye compared with the previous period,and the degree of spherical equivalent and the axial length of the eye were still significantly advanced(P≤0.05).③After treatment,the naked eye visual acuity,the degree of spherical equivalent,the amount of axial growth and the rating of bipolar disorder in the treatment group were significantly different from those in the control group(P≤0.05),while the length of axial and corneal curvature were not significantly different from those in the control group(P>0.05).④During the course of the study,no adverse effects occurred in both the treatment and control groups,and there were no abnormalities in the safety indicators before and after treatment.Conclusion:①He’s acupuncture-moxibustion three removing obstruction method can achieve good control of naked eye visual acuity,spherical equivalent,and axial growth in adolescents with low myopia and bipolar disorder.②There was a trend,but the difference was not significant,for improvement of corneal curvature in adolescents with low-grade myopia and bipolar disorder in the short term by treatment with He’s acupuncture-moxibustion three removing obstruction method.③No adverse effects and adverse events were observed during the course of the study by Shigella three way assay,which was safe.
