Background:In recent years,with the epidemic of Novel coronavirus SARS-CoV-2(COVID-19),people’s psychological stress is so high that there are more mental illnesses,and it has been clinically observed that most patients often complain of insomnia,but they are accompanied by different degrees of emotional disorders,which also have a certain impact on the quality of life of patients.Most Western drugs have serious adverse reactions and side effects,and are prone to dependence.Traditional Chinese medicine,on the other hand,is mainly based on dialectical treatment,and has unique advantages in the treatment of sleeplessness.Therefore,to clarify the distribution of TCM symptom types and quality of life status of insomnia patients with anxiety and depression as early as possible is very important.Objective:To explore the general situation of insomnia patients with anxiety and depression,sleep quality,anxiety degree,depression degree and distribution characteristics of TCM symptom type,evaluate their quality-of-life status,and study the correlation between variables.In order to provide more comprehensive data reference for clarify dialectical therapy and finding ways to improve quality of life,so as to improve clinical efficacy.Methods:Insomnia patients with anxiety and depression from the Department of Encephalopathy of Dongzhimen Hospital of Beijing University of Chinese Medicine from December 2021 to December 2022 were collected,the collected scale information was entered into Excel to establish a database,and SPSS 25.0 statistical software was used to descriptive statistics on general data,and the normal distribution of the measurement data and the variance were homogeneous using t-test or one-way ANOVA;Nonnormal distribution,uneven variance with nonparametric tests,and correlation analysis using the Person correlation coefficient.Results:1 A total of 195 insomnia patients with anxiety and depression were included in the study,with a male-to-female ratio of 1:1.90.The junior group accounted for 1.03%,the youth group accounted for 33.85%,the middle-aged group accounted for 57.95%,the elderly group accounted for 7.18%,and the average age of men was slightly higher than that of women.Mainly married patients,living in urban areas,engaged in intellectual work,bachelor degree or above,too much thinking and a balanced diet,most of the patients have no chronic history,no history of smoking and alcohol,and no physical exercise.2 The proportion of mild,moderate and severe insomnia among the included patients was 1:3.8:2.1;the proportion of possible anxiety,affirmative anxiety,obvious anxiety and severe anxiety was 1:2.4:2.5:1.1;and the proportion of depressive state,depression and major depression was 2.4:4.7:1.There was no significant difference in PSQI score between men and women and between causes of insomnia(P>0.05),but the difference between HAMA score and HAHD score was statistically significant(P<0.05).The difference in PSQI score between different age groups was statistically significant(P<0.05),but the difference between HAMA score and HAHD score was not statistically significant(P>0.05).3 Among the TCM certificate types,liver depression transforming into fire type were the most,accounting for 34.9%,followed by Yin-deficiency fire-hyperactivity type,accounting for 31.3%,and the number of internal disturbances of phlegm-heat was the smallest,accounting for 7.2%.The difference in the distribution of TCM symptom types between men and women was significant(P<0.05).However,the difference in distribution of TCM symptom types between different age groups or other demographic data was not significant(P>0.05).4 Patients scored the highest in the environmental domain and the lowest in the physiological domain,with no significant difference between men and women(P>0.05),and significant differences between age groups in scores and total quality of life scores in the remaining dimensions except the social domain dimension(P<0.05).Except for the physiological domain,the scores of the remaining dimensions and the total quality of life score had significant differences between the causes of insomnia(P<0.05).5 Correlation analysis showed that there was a significant positive correlation between PSQI,HAMA and HAHD scores(P<0.05).The difference in insomnia and anxiety between TCM types was significant(P<0.05).The difference in depression between TCM types was not significant(P>0.05).The scores of physiological,psychological,environmental and quality of life were significantly negatively correlated with HAMA,HAMD,and PSQI scores(P<0.05),while the social domain was only negatively correlated with the HAHD score(P<0.05).The difference between TCM certificate type in all dimensions of WHOQOL-BREF scale was not significant(P>0.05)Conclusion:1 Women,middle-aged,married,highly educated,and intellectual workers are mostly insomnia susceptible people with anxiety and depression.Insomnia is concentrated in moderate to severe,anxiety is concentrated in obvious anxiety and possible anxiety,depression is concentrated in depression and depressive states,women have a higher degree of anxiety and depression than men,the older they are,the worse the quality of sleep,and insomnia is more severe due to family friction.2 Traditional Chinese medicine certificates are mainly liver depression transforming into fire type and Yin-deficiency fire-hyperactivity type,male is mainly Yin-deficiency firehyperactivity type and women are mainly 1 liver depression transforming into fire type.3 Patients scored highest in the environmental domain,the lowest in the physiological domain,and middle-aged and elderly patients and those with insomnia due to family friction had the worst quality of life.4 The degree of insomnia,anxiety,and depression have a significant positive correlation with each other.5 The degree of insomnia in patients with sputum fever is heavier,and the degree of anxiety in patients with heart deficiency and timidity is more serious.6 The score of the social field was only negatively correlated with the degree of depression,and the total score of physiological,psychological,environmental and quality of life was negatively correlated with insomnia,anxiety and depression.
