| Part I: The research of psychological status in patients with CG and their relation to the syndrome types of TCMObjective: The purpose of this study is to investigate the relevant factors of psychological problems and explore the distribution of TCM syndrome types of patients with CG. All these work could provide theory basis for better treatment of CG patients.Methods: Since March 2013 to December 2014, we used questionnaires to investigate the psychological status of 446 patients with CG. The questionnaires include Hospital anxiety and depression scale(HAD), Pittsburgh Sleep Quality Index(PSQI).Results: Recycling of 446 valid questionnaires has 95.09 percent efficiency. 101 patients(22.65%) had symptoms of anxiety and 104 patients(23.32%) had symptoms of depression. There were 62 patients(13.90%), who had two kinds of symptoms at the same time. 158 patients( 35.43%) had symptoms of insomnia. Multivariate regression analysis found the incidence of anxiety and depression in CG patients were no correlated with gender and age(p>0.05), but were related to education(p<0.05), occupation(p<0.05) and Duration of disease(p<0.01). Both the quality of sleep and the distribution of syndromes are related to Whether associated with anxiety and depression(p<0.01).Conclusions: The anxiety, depression and insomnia of the CG patients was significantly higher prevalence, and related to multiple factors. The most common syndrome of these patients with anxiety and depression was liver qi stagnation concurrently with dampness heat in spleen and stomach.Part II: Clinical study on HUA SHI JIE YU decoction treating thesyndrome of liver qi stagnation concurrently with dampness heat inspleen and stomach of CG accompanied with anxiety-depressionObjective: The purpose of this study is to observe the clinical effect and safety of which on the basis of western medicine routine treatment combined with HUA SHI JIE YU decoction on patients with CG accompanied with anxiety–depression and to provide a more efficient and safer clinical treatment scheme for those special patients.Methods: a randomized controlled study method, 42 cases with CG accompanied with anxiety-depression patients were divided into treatment group and control group with 21 cases in each group. The control group was administered with simple western medicine of CG and flupentixol and melitracen tablets(Dailixin), the treatment group was treated with same western medicine of CG plus HUA SHI JIE YU decoction, the course of treatment was 4 weeks. The clinical effect and safety of the medicine were evaluated with the score of TCM syndromes, anxiety-depression, quality of life and sleep before and after the treatment. In addition, Pre-treatment and post treatment, we would respectively finished the tests of blood and urine routine, liver and kidney function.Results: 1. TCM syndromes: As Compared with before treatment, the patients in the two groups have a significant decrease in the score of TCM syndromes(p<0.05);the treatment group and the control group, epigastric fullness, pain in chest and hypochondrium, nausea and vomiting, depression, gastric calor symptoms difference were statistical1 y significant(p<0.05), treatment group better;However, there was no difference among the other three symptoms(p>0.05).2. Quality of life : As Compared with before treatment, the patients in the two groups have a significant increase in the score of quality of life(p<0.05);the treatment group for Role-Physical, Vitality, Physical Functioning, General Health, improvements were superior to the control group,there were statistical differences between two groups(p<0.05);However, there was no difference among the other four dimensions(p>0.05).3. Quality of sleep: Before and after the treatment, the two groups could decrease the score of sleep quality(p<0.05);But there was no difference between the two groups after the treatment(p>0.05).4. The efficacy of anxiety-depression: Before and after the treatment, the two groups could decrease the score of anxiety-depression(p<0.05);But there was no difference between the two groups after the treatment(p>0.05).5. Untoward effect:There was no serious adverse events recorded in the treatment group and the control group during the whole study.Conclusions: The treatment of HUA SHI JIE YU decoction is beneficial to alleviate anxiety, depression, improve quality of life and sleep in CG patients, its worth further clinical promotion. |
