| Objective:This study adopts two parts:cross-sectional study and literature study.Through cross-sectional study,the clinical data of insomnia patients are collected,and the sleep and gastrointestinal conditions of insomnia patients are understood.Through correlation analysis,the correlation between insomnia and gastrointestinal symptoms is found out.Through literature study,the intervention effect of acupuncture on insomnia and functional gastrointestinal diseases is systematically evaluated,which provides reference for clinical diagnosis and treatment.Methods:This study consists of two parts:cross-sectional study and literature study.Questionnaire survey was published on the online platform,and the data of insomnia patients from August 2022 to April 2023 were collected.The database was established in Excel,and descriptive analysis and correlation analysis were carried out by SPSS 26.0.The literature research part searches PubMed,EMbase,Cochrane Library,Web of Science,National Knowledge Infrastructure Database(CNKI),Chinese Academic Journal Database(CSPD),Chinese Sci-tech Journal Database(CCD)and Chinese Biomedical Literature Database(CBM)by computer.Randomized controlled trials(RCT)related to acupuncture treatment of insomnia complicated with functional gastrointestinal diseases were searched.Two researchers independently conducted literature screening,Meta extraction and bias risk assessment.Finally,a meta-analysis was conducted by using RevMan 5.3 software.Results:Clinical cross-sectional study part:1 General information of insomnia patients:This study included 260 insomnia patients,including 166 female insomnia patients and 94 male insomnia patients,the ratio of male to female was 1:1.64.The youngest is 18 years old and the oldest is 74 years old,mainly distributed in the age group of 26-35 years old.Bachelor degree,183 people,accounting for 70.4%.Private enterprises are the main work units,with 120 employees,accounting for 46.2%,followed by students,accounting for 21.2%.2 Sleeping situation of insomnia patients:According to the PSQI score,the lowest score of PSQI is 8 points,and the highest score is 21 points.90.4%reported poor or very poor sleep quality,78.8%insomnia patients slept longer,91.9%insomnia patients slept less than 7 hours,80%insomnia patients had low sleep efficiency,61.6%insomnia patients had frequent sleep disorder symptoms,and 70%insomnia patients said.According to ISI score,there are 91 patients with mild insomnia,133 patients with moderate insomnia and 36 patients with severe insomnia,accounting for 35%,51.2%and 13.8%respectively.3 Gastrointestinal symptoms of insomnia patients:The incidence of gastrointestinal symptoms in insomnia patients is 99.6%,and the top three gastrointestinal symptoms are abdominal distension,increased exhaust and abdominal pain.The top three scores of gastrointestinal symptoms are abdominal distension,increased exhaust and acid reflux.4 Correlation between sleep quality and gastrointestinal symptoms:the occurrence of heartburn is positively correlated with PSQI score,among all factors,the factor of falling asleep is positively correlated with the occurrence of abdominal pain,heartburn,acid reflux,abdominal tightness,abdominal distension and belching,while the sleep efficiency is not correlated with the occurrence of gastrointestinal symptoms,and the correlation between other PSQI factors and gastrointestinal symptoms is low or very low;The PSQI score and various factors are positively correlated with the severity of gastrointestinal symptoms,and the severity of loose stool or hard stool is highly correlated with the PSQI score.The severity of increased defecation,loose stool and hard stool is moderately correlated with sleep quality,with abdominal pain,heartburn,acid reflux,tightness,nausea,vomiting,abdominal sound,abdominal distension,belching,increased exhaust,increased defecation,loose stool and hard stool.The severity of loose stool and hard stool is moderately related to the actual sleep time,the severity of belching,increased exhaust,increased defecation and loose stool is moderately related to sleep disorder,the severity of loose stool is moderately related to hypnotic drug use,and the severity of abdominal pain,increased defecation,loose stool and hard stool is moderately related to daytime dysfunction.5 Correlation between the severity of insomnia and gastrointestinal symptoms:among 91 patients with mild insomnia,the top three gastrointestinal symptoms were abdominal distension,increased exhaust and abdominal pain,accounting for 68.10%,62.60%and 60.40%respectively;Among 133 patients with moderate insomnia,the top three gastrointestinal symptoms were increased exhaust,abdominal pain and abdominal distension,accounting for 80.50%,80.50%and 79.70%respectively.Among 36 patients with severe insomnia,the top three gastrointestinal symptoms were belching,heartburn and abdominal sound,accounting for 100%,100%and 94.40%respectively.Through correlation analysis,it is found that the occurrence of gastrointestinal symptoms is positively correlated with the severity of insomnia,and the correlation degree is low or very low;There is a positive correlation between the severity of insomnia and gastrointestinal symptoms.The severity of abdominal pain,decreased defecation,increased defecation and loose stool is moderately correlated with the severity of insomnia,and the correlation coefficient of increased defecation is the largest,which is 0.663,indicating that the influence between the severity of increased defecation and the severity of insomnia is the most important.Literature research part:Through literature search,a total of 1902 related studies were initially obtained.After screening,9 randomized controlled trials were included for systematic evaluation,including 8 Chinese literatures and 1 English literature,involving 984 patients,492 cases in the experimental group and 492 cases in the control group.Among them,there are 4 articles on acupuncture compared with western medicine and 5 articles on acupuncture combined with massage compared with acupuncture.Acupuncture compared with western medicine group,the results showed that the PSQI score of acupuncture group after treatment was significantly lower than that of western medicine group,and the difference was statistically significant(P<0.05).We further compared acupuncture and acupuncture combined massage therapy,and the results showed that the PSQI score of acupuncture combined massage therapy was significantly lower than that of acupuncture group,and the difference was statistically significant(P<0.05),A randomized controlled trial reported gastrointestinal related indexes,involving 60 cases.The experimental group was treated with acupuncture,and the control group was alprazolam.After treatment,the levels of the two groups were significantly improved compared with those before treatment,with a significant difference(P<0.05),and the improvement in the acupuncture group was more obvious.Conclusions:This study is divided into two parts:cross-sectional study and literature system evaluation.The conclusions are as follows:1 Sleep quality has a moderate positive correlation with heartburn and abdominal distension,and the severity of loose stool or hard stool has a high positive correlation with sleep quality;2 The most common gastrointestinal symptoms in patients with mild and moderate insomnia are abdominal distension,increased exhaust and abdominal pain;Among patients with severe insomnia,the top three gastrointestinal symptoms are increased exhaust,acid reflux and abdominal distension;3 The severity of insomnia is positively correlated with the occurrence and severity of gastrointestinal symptoms,and the severity of abdominal pain,decreased defecation,increased defecation and loose stool is moderately correlated with the severity of insomnia,among which the severity of increased defecation is the most important.4 In terms of literature system evaluation,acupuncture and acupuncture combined with massage treat insomnia and functional gastrointestinal diseases.In terms of improving sleep quality,acupuncture combined with massage is better than acupuncture alone,but it is not clear whether it can improve gastrointestinal symptoms.5 The literature included in systematic review research is limited,and more high-quality clinical trial data are needed in the future to provide more powerful and comprehensive evidence. |
