Clinical Observation On The Treatment Of Abnormal Uterine Bleeding In Adolescence With Kidney Deficiency And Liver Depression By Yishen Tiaogan Prescription Combined With Progesteron

Objective: To study the effect of Yishen Tiaogan decoction combined with Yimaxin(capsule)on the treatment of abnormal uterus bleeding of kidney deficiency and liver depression type(Abnormal uterine bleeding-ovulatory dysfunction,AUB-O)by observing the changes of bleeding volume,hemostasis time,TCM syndromes scorel,basal body temperature(BBT)and sex hormone level before and after treatment.It is proved that Yishen Tiaogan decoction combined with Yimaxin in the treatment of AUB-O of kidney deficiency and liver depression type provides evidence for the safety and practicability of clinical application of Yishen Tiaogan decoction.Scheme:The 60 outpatient cases that met the inclusion criteria were divided into control and observation groups.Control group: progesterone capsule,oral,100 mg / time,once every 12 hours,after 15 days on the day of enrollment,the next time to take is the 11 th day of the menstrual cycle,followed by 15 days,for a total of 3 menstrual cycles.Observation group:Chinese medicine decoction 15 payments,water decoction,1 payment per day,after breakfast and dinner,specific dosage: bleeding period,take Chinese medicine decoction,one payment per day,water decoction,400 ml,take after breakfast and dinner,take for 15 days,use kidney to regulate liver addition and subtraction.At the same time,progesterone capsules were taken in combination(the usage and cycle were the same as those of the control group).The observation group and the control group had a menstrual cycle from the first day of this menstrual period to the first day of the next menstrual period,and the course of treatment was 3 menstrual cycles.If there are dizziness,drowsiness and other side reactions during the treatment,it is necessary to stop the drug and take 10 mg vitamin B6 tablets.After 6 months of follow-up after the end of the course,the age distribution and disease degree of the two groups were observed and analyzed before treatment,and after the three courses of treatment,the hemostatic efficacy,TCM syndrome score,hemoglobin value,progesterone value,BBT and comprehensive efficacy of the disease were recorded and compared between the two groups.Statistical methods: SPSS26.0statistical analysis software was used for analysis.Results: 1.The hemostasis time of the two groups was compared,and there was no significant difference in hemostasis efficacy between the two groups(P>0.05).2.Comparing the scores of medical syndrome in the two groups,both treatments could improve the symptoms of patients,and the decline in points after treatment of the two groups was analyzed by independent sample t-test,which showed that the observation group was significantly better than the control group in improving the clinical symptoms of renal deficiency and liver depression in adolescent AUB-O patients(P<0.05).3.It can be seen from the independent t test that the hemoglobin P between the two groups before treatment > 0.05,and the hemoglobin P between the two groups after treatment < 0.05,indicating that it is more effective to improve hemoglobin in the observation group.4.According to the independent t-test,the progesterone value of the observation group after treatment was P<0.05 compared with the control group,indicating that the observation group had a more significant effect on improving the level of progesterone in the middle luteal period.5.Compared with the efficacy of restoring ovulation after treatment between the two groups,there were 28 cases of ovulation and 2 cases of anovulation in the observation group,21 cases of ovulation and 9 cases of anovulation in the control group.6.The changes of BBT in the two groups were compared,and the monophasic decrease of BBT in the observation group was significantly higher than that in the control group(P<0.05)through rank sum test analysis.7.Comparing the comprehensive efficacy of the two groups,18 cases of rapid acting,7 cases of obvious effect,3 cases of effective and 2 cases of ineffectiveness in the observation group;The control group had 10 cases of rapid acting,7 cases of apparent effect,10 cases of effective and 3cases of ineffectiveness.Conclusion: 1.The combination of progesterone can reduce the hemostasis time,effectively improve hemoglobin,regulate progesterone value,ovulation function and BBT in the middle luteal period,improve TCM symptoms,and have better efficacy of kidney and liver regulation.2.This study has confirmed the safety of the combination of kidney and liver regulation formula with progesterone,indicating that traditional Chinese medicine combined with western medicine is safe and reliable in the treatment of adolescent AUB-O.
