| Objective:Through the patients with perianal abscess disease and treatment related data,in patients with perianal abscess the five human qualities research,analysis of the basic information of various types of patients with perianal abscess difference,pathogenesis,treatment and the situation,according to the physiological characteristics of five human qualities,provide more theoretical guidance.Methods:Collect the data of 168 patients with perianal abscess hospitalized in the First Hospital of Traditional Chinese Medicine in Changde city from June 2020 to June 2022,the five human qualities of TCM constitution classification,concluded that the general situation of various constitution(gender,age,season,BMI),risk factors,days,hospital inflammatory indicators,TCM differentiation classification,abscess type,surgical selection,hospital days,after the data,and rehabilitation were analyzed statistically by using SPSS24.0 software.Results:Among the 168 patients included in the study were 21 wood shaped person,42 fire shaped person,65 earth shaped person,17 metal shaped person and 23 water shaped person.The specific results of each type of constitution are as follows:(1)Wood shaped person:The prevalence of perianal abscess in women was slightly higher than that in men,mainly 21-50 years old,accounting for 81%.The incidence was mainly autumn and winter(90.5%),and the difference between men and women was not statistical significant(P> 0.05),and BMI was mostly normal.The main risk factors for wood shaped person were eating spicy food(76.2%),constipation(38.1%))and diarrhea(33.3%).76.2% of wood shaped person were admitted within 0-3 days of onset and 19% within 4-7 days of onset.Among the inflammatory indicators,WBC,L,and N were mostly normal,and 38.1% of the patients had high CRP.TCM syndrome was mainly thermal and toxic syndrome,accounting for 76.2%,and western medicine classification was mainly simple low abscess,accounting for 71.4%.About76.2% of patients used primary incision and radical resection,the average hospitalization of 5-7days,about 14.2% of patients formed anal fistula,no abscess recurrence was found temporarily,and the recovery was good.(2)Fire shaped person:The incidence of men was significantly higher than women,with50% of patients aged 21-50 years,followed by 10 patients aged 6-17 years,accounting for23.8%,more than in autumn and winter.BMI suggested some overweight.The main risk factors for fire pedestrians are eating spicy food,constipation,smoking and betel nut chewing.57.1% of patients were admitted within 0-3 days of disease onset,28.6% were admitted within 4-7 days of disease onset,Significsignificantly high significantly ocytes in 83.3% of patients,85.3%neutrophils,CRP was high in 83.3%,92.9%,Complex low abscesses accounted for 42.9%,Simple high abscesses accounted for 38.1%,Complex high abscesses accounted for 19%,Of 34 patients undergoing primary hanging line surgery,At 81%,A total of 5 patients were operated on for open pus discharge,At 11.9%,62% were hospitalized for 8-11 days,23.8% were hospitalized for more than 11 days,The recurrence accounted for 4.8%,14.2% formed anal fistula in 14.2%.(3)Earth shaped person:Men were significantly higher than women,with 46 patients aged21-50 years,accounting for 70.8%,and 49.2% of patients aged 18-20 years.More than spring /summer onset and overweight with 21.5%BMI.The main risk factors are cigarette smoking,constipation,chewing betel nut,and eating spicy food.Thirty-nine patients were admitted within4-7 days of onset,accounting for 60%;11 patients within 1-11 days,16.9%;WBC,N,and CRP were high.TCM syndrome differentiation is mainly hot and concentrated,accounting for 64.6%and 30.8%.Most of them are complex low abscesses or high abscesses,most of which are primary line radical resection or primary incision radical resection is used.The hospitalization time is long,the recurrence rate is 4%,more people form anal fistula,and the fistula rate is about26%.(4)Metal shaped person:The proportion of patients with anal carbuncle was equal,35.3%of patients aged 18-20 and 47.1% of patients aged 21-50.More than spring and summer onset,BMI was more normal.The main risk factors for disease are constipation and spicy food.58.8%were admitted 0-3 days after onset,some 4-7 days,and a few over 11 days.Most of the inflammatory indicators were mildly elevated.For syndrome differentiation,a few were fiery toxic syndrome,mostly simple low abscesses or high abscesses.76.5% of the patients were treated with primary line radical resection,and the hospitalization time was not long.Two people formed anal fistula,and the fistula rate was 11.8%,and no recurrence was seen.(5)Water shaped person:Patients with anal carbuncle were significantly higher in males than females,and 73.9% of patients were over 50 years old.More than in spring and summer,BMI overweight accounted for 11%.The risk factors are mainly diarrhea and smoking.69.6% of patients were admitted after more than 11 days of disease onset,and 21.7% were admitted within8-11 days of disease onset.The inflammatory indicators were mostly normal.TCM syndrome differentiation was Yin deficiency syndrome,accounting for 78.3%,8.7%,17.3% of patients,73.9%,78.3% of patients hospitalized more than 11 days,6 cases of anal fistula,accounting for26.1%,No abscess recurrence was found.Conclusion:Five elements of the constitution of the fire pedestrian,local pedestrian constitution for the high incidence of anal carbuncle crowd;Women are prone to anal caruncle,And the chance of radical resection is minimum;Fire pedestrian constitution of young children prone to anal caruncle,Eating spicy food is its main risk factor,The recurrence rate is the highest;Local pedestrian physique in the prime of age multiple anal carbuncle,Obesity is an independent risk factor for its pathogenesis,The recurrence rate is that the fistula rate is high;Gold pedestrian constitution is prone to low simple abscess,The lowest fistula rate;Water pedestrian constitution to the middle-aged and elderly people with high incidence of anal carbuncle,And it is easy to delay the medical treatment time,Deldelay the illness,The slowest recovery,The fistula rate is the highest;The earlier the presentation is,The disease is still in a simple stage,The probability of primary radical resection is high,The faster the recovery is made,The lower the fistula rate and the recurrence rate are. |
