Objective: In this study,we compared the clinical effects of the dual wavelength laser vaporization of the prostate with the transurethral plasmakinetic resection of the prostate to evaluate the advantages of the clinical effectiveness and safety of the dual wavelength laser vaporization of the prostate in the surgical treatment of benign prostatic hyperplasia.Through research and statistics,we analyzed whether there were advantages in operation time,bleeding volume,postoperative bladder washing time,indwelling catheter time,postoperative hospitalization time,and postoperative complications of the dual wavelength laser vaporization of the prostate compared with the transurethral plasmakinetic resection of the prostate,so as to provide help for choosing the appropriate operation method for the clinical treatment of benign prostatic hyperplasia.Methods: 1.In order to compare the effectiveness and safety of TUPKRP and DWLV as a whole,we selected the RCTs and non-RCT of TUPKRP and DWLV that met the requirements by retrieving Pub Med,CNKI,VIP and Wanfang Database,and conducted quality evaluation and data extraction for the literature.Finally,we used statistical software Rev Man5.3 to complete the Meta-analysis.2.In this study,the data of all patients who were hospitalized in the Department of Urology and underwent prostate surgery from January 2021 to December 2022 were exported in the Medical Record Room of Changsha Central Hospital,and then a random number table was used to select60 patients after TUPKRP and 60 patients after the dual wavelength laser vaporization of the prostate(DWLV),totaling 120 patients in total.Two groups of patients were compared with IPSS,PVR,Qo L,and Qmax before and three months after surgery to evaluate effectiveness and operation time,bleeding volume,postoperative bladder washing time,indwelling catheter time and postoperative hospital stay and postoperative complications to evaluate safety.Results: 1.Eventually,seven articles were included,and a total of 601 patients were included in the study(297 cases in DWLV group and 304 cases in TUPKRP group).Meta-analysis results showed that: 1.In terms of effectiveness,DWLV compared with TUPKRP procedure had statistical differences in Qmax,IPSS score and QOL score.DWLV procedure played a more dominant role in the treatment effectiveness of BPH and the symptom relief degree of LUTS after surgery was more obvious.2.In terms of safety,the operation time,postoperative bladder washing time,duration of indwelling catheter and hospital stay of DWLV operation mode are shorter,and the intraoperative blood loss is less,which indicates that DWLV operation mode has the advantages of lower risk,higher efficiency,and quicker recovery and less pain for patients after operation.In terms of complications,there was no statistical difference in the incidence of urinary infection between the two surgical methods.The incidence of postoperative secondary bleeding complications was lower in DWLV surgical method than in TUPKRP surgical method,indicating that the safety of DWLV surgical method was higher than that of TUPKRP surgical method,and the risk of postoperative complications was effectively reduced.2.TUPKRP and DWLV can both significantly reduce the IPSS score,PVR,and Qo L score,and significantly increase Qmax.There is no significant difference in effectiveness between the two groups in IPSS score,PVR,Qo L score,and Qmax after surgery,indicating that both two surgical methods can effectively improve the LUTS symptom and life quality of patients.In terms of safety,the operation time,bleeding volume,postoperative bladder washing time,indicating that the patients in the DWLV group had lower operation risk during the operation,and recovered faster after the operation,thus enabling them to have a better quality of life and medical experience.Moreover,the risk of postoperative complications for patients in the DWLV group was significantly lower than that in the TUPKRP group,indicating that DWLV was more advantageous than TUPKRP in reducing the postoperative complications of BPH.Conclusion: In the treatment of BPH patients,dual-wavelength laser vaporization can effectively shorten the operation time,reduce the time of postoperative indwelling catheter and continuous bladder irrigation,make the patients recover faster after surgery,significantly reduce the amount of intraoperative bleeding,and reduce the incidence of postoperative secondary bleeding and urinary tract infection.Dual-wavelength laser vaporization has higher surgical safety than plasma resection,with faster postoperative recovery,less pain and lower risk of postoperative complications.It provides a better surgical option for BPH patients.
