purpose : To collect the medical records of patients who were hospitalized in the Department of Rheumatology and Rheumatology Rehabilitation of the Affiliated Hospital of Liaoning University of Traditional Chinese Medicine from September 2012 to September2022 and those who were diagnosed as dermatomyositis in the Department of Rheumatology from September 2020 to September 2022.Through retrospective analysis of the collected medical records,the distribution of syndrome types and medication rules of dermatomyositis were summarized,in order to provide some clinical ideas and medication experience for clinical treatment of dermatomyositis.Material and method : This study included the clinical data of patients who were hospitalized in the Department of Rheumatology and Rheumatology Rehabilitation of the Affiliated Hospital of Liaoning University of Traditional Chinese Medicine from September2012 to September 2022 and those who were diagnosed as dermatomyositis in the outpatient department of rheumatology from September 2020 to September 2022.EXCEL2016,SPSS Statistics25.0 software and IBM SPSS Modeler 18.0 software were used to analyze the syndrome distribution,drug frequency and association rules of the cases that met the inclusion and exclusion criteria,and the data were mined.The syndrome distribution characteristics and medication rules of dermatomyositis were obtained.Results :1.A total of 82 patients with dermatomyositis were included in this study,including 26 male patients and 56 female patients,with a male to female ratio of 1 : 1.93;the mean age was 51± 15 years.2.The frequency of TCM syndromes from high to low is the syndrome of heat toxin(29.27 %);spleen and kidney deficiency syndrome(23.17 %);damp-heat obstruction syndrome(21.95 %);phlegm and blood stasis syndrome(20.73 %);cold dampness obstruction syndrome(4.87 %).3.In this study,a total of 82 prescriptions were counted,involving 139 flavors of traditional Chinese medicine,with a total frequency of 1139 times.Licorice,Poria,tangerine peel,Astragalus,Atractylodes,Angelica,Coix seed,Pinellia,and Scutellaria are high-frequency drugs for the treatment of this disease.The drug categories were treated in a unified and standardized manner.This study involved a total of 15 types of drugs,commonly used drugs such as tonic drugs,heat-clearing drugs,water-clearing and dampness-percolating drugs,anti-surface drugs,and blood-activating and stasis-removing drugs.From the perspective of medicinal properties,cold drugs are used the most,ranking from high to low as cold,warm,flat,hot and cool.The taste of medicine is mainly sweet,bitter and pungent.From the analysis of drug meridian,the majority of drugs belong to liver meridian,lung meridian,kidney meridian and spleen meridian.4.Cluster analysis was performed on the top 25 drugs used frequently.The drugs can be divided into three groups,class 1 : tangerine peel,pinellia,bupleurum,dandelion,and windbreak.Class 2 : Forsythia,Atractylodes,Alisma,Licorice,Scutellaria,Dioscorea.Roumai class 3 : Ophiopogon japonicus,coix seed,yam,white fresh bark,cassia twig.Category 4 : Caulis Spatholobi,Atractylodes,Codonopsis,Poria,Astragalus,Achyranthes,Angelica,Chuanxiong,Paeonia.Correlation analysis was performed on 25 traditional Chinese medicines with a frequency of medication ≥ 15.The second-order correlation results showed that the highest support was Chenpi-Banxia,the highest third-order correlation support was Baizhu + Gancao-Fuling,and the highest fourth-order correlation support was Huangqi + Fuling + Gancao-Baizhu.Conclusion :1.Clinically,patients with heat toxin syndrome,spleen and kidney deficiency syndrome are the most common,followed by damp heat syndrome,phlegm and blood stasis syndrome,cold dampness syndrome is less.2.Medication treatment should be tonify deficiency,combined with heat-clearing drugs,relieving drugs,water-clearing and dampness-percolating drugs,blood-activating and stasis-removing drugs to strengthen the body and eliminate pathogens.3.Commonly used drug combinations for tangerine peel-Pinellia and Astragalus + Poria +Glycyrrhiza-Atractylodes based on the addition and subtraction.
