| Purpose:To analyze the differences in the clinical characteristics and the distribution of TCM syndrome elements in patients with primary aldosteronism(PA)with different serum potassium levels,and to provide new ideas and solutions for the diagnosis and treatment of PA with traditional Chinese medicine.Material and method:From January 2019 to December 2022,a total of 80 patients with PA diagnosed by laboratory examination,imaging examination and diagnostic tests for primary aldosteronism in the Department of Endocrinology and Department of Urology,Affiliated Hospital of Liaoning University of Traditional Chinese Medicine were selected,and the general clinical data of the patients were collected.Data,physical and chemical examination results,imaging examination results,and TCM syndrome elements were divided into hypokalemia group and normokalemia group according to the difference in blood potassium level,and the data of the two groups were compared and analyzed.Results:1.The body mass index of the hypokalemia group was significantly lower than that of the normal blood potassium group(P<0.01);and the age,24-hour mean systolic blood pressure,fasting blood glucose,glycosylated hemoglobin,urinary microalbumin/creatinine ratio,orthostatic aldosterone level and plasma aldosterone/renin activity ratio were significantly higher than those of the normal blood potassium group(P<0.05).2.The cases of cardiovascular disease,cerebrovascular disease and kidney disease in the hypokalemia group were significantly higher than those in the normal potassium group(P<0.05).3.The symptoms of fatigue,palpitations,spontaneous sweating,limb numbness,and muscle atrophy in the hypokalemia group were significantly higher than those in the normokalemia group(P<0.05);the symptoms of bitter mouth and poor sleep in the normokalemia group were significantly higher than those in the hypokalemia group Blood group(P<0.05).4.The number of adrenal adenomas in the hypokalemia group was significantly higher than that in the normokalemia group(P<0.05);the number of normal adrenal glands in the normokalemia group was significantly higher than that in the hypokalemia group(P<0.01).5.In the syndrome elements of TCM,the distribution of hypokalemia group was kidney>liver>spleen>heart,and the kidney was significantly higher than that of normal blood potassium group(P < 0.05);The distribution of normal blood potassium group was liver>kidney>spleen>heart,and liver was significantly higher than hypokalemia group(P<0.05).6.The distribution of solid syndrome elements in hypokalemia group was phlegm >blood stasis > Yang hyperactivity > qi stagnation,phlegm and blood stasis were significantly higher than normal potassium group(P<0.05);The distribution of normal blood potassium group was Yang hyperactivity > phlegm > blood stasis > qi stagnation,and Yang hyperactivity was significantly higher than hypokalemia group(P<0.01).Among deficiency syndromes,the distribution of hypokalemia group was qi deficiency > Yang deficiency > Yin deficiency >blood deficiency,and qi deficiency and Yang deficiency were significantly higher than those in normal blood potassium group(P<0.01).The distribution of normal blood potassium group was Yin deficiency > Qi deficiency > blood deficiency > Yang deficiency,and Yin deficiency was significantly higher than hypokalemia group(P<0.01).Conclusion:1.The blood potassium level of PA patients may be related to age and disease development;The clinical symptoms of PA patients with hypokalemia are serious.It is recommended to monitor the blood pressure level and blood sugar in daily life.2.Hypokalemia may have predictive effect on the damage of target organs such as heart,brain and kidney in PA patients.3.TCM diagnosis and treatment of PA is generally based on the treatment principle of reinforcing deficiency and purging deficiency.PA patients with normal blood potassium can be treated from the perspective of Yin deficiency and Yang hyperactivity of the liver,and Traditional Chinese medicine prescription,which nourishes Yin and reduces fire,has a good effect.PA patients with hypokalemia can be treated from the Angle of renal "adenoma".In the elimination of "adenoma" at the same time,the Chinese herbal prescription of Yiwenyang has a good clinical effect. |
