Objective: To observe the clinical study of Fuzi Decoction in the treatment of cervical radiculopathy(Shaoyin disease)based on six channels differentiation. Methods: In the first part,80 patients admitted to the Spine Department of the First Affiliated Hospital of Hunan University of Chinese Medicine from December 1,2021 to December 1,2022 were recruited and divided into treatment group and control group by simple random sampling, with 40 cases in each group. The treatment group was treated with aconite decoction, while the control group was treated with celecxib capsules and mecobalamine tablets for 2 weeks.Visual analogue scale(VAS),JOA score of cervical spondylosis,TCM syndrome score and clinical efficacy were compared before and after treatment. After 2 months of discontinuation, recurrence rates were again compared between the treatment and control groups. The second part explores the potential molecular mechanism of Fuzi decoction in the treatment of radiculopathy based on network pharmacology and molecular docking techniques.Results:1.In the first part,80 patients admitted to the Spine Department of the First Affiliated Hospital of Hunan University of Chinese Medicine from December 1,2021 to December 1,2022 were recruited and divided into treatment group and control group by simple random sampling,with40 cases in each group. The treatment group was treated with aconite decoction combined with Celecoxib capsules combined with mecobalamine tablets, while the control group was treated with celecoxib capsules and mecobalamine tablets. The duration of the medication was 2 weeks.Visual analogue scale(VAS),cervical JOA score,TCM syndrome score and clinical efficacy were compared before and after treatment. After 2 months of discontinuation, recurrence rates were again compared between the treatment and control groups. The second part explores the potential molecular mechanism of Fuzi decoction in the treatment of radiculopathy based on network pharmacology and molecular docking techniques.2.Through network pharmacological analysis, the potential effective components of Fuzi decoction in the treatment of radiculotic cervical spondylosis were obtained, such as Frutinone A,Fumarine,beta-sitosterol,kaempferol,etc.It mainly involves the expression of TNF signaling pathway,IL-17 signaling pathway and c-type lectin receptor signaling pathway, which are closely related to inflammatory targets such as IL6,IL1 B,PTGS2,CASP3,CASP1 and VCAM1.Molecular docking technology further verified that the binding energy of eugenone A with PTGS2,CASP1,β-glutsterol with PTGS2,CASP3,CASP1 and other targets was high, suggesting that eugenone A may be a key active component of drug efficacy.Conclusion:1.Fuzi Decoction has a clear clinical effect in the treatment of radiculopathy of cervical spondylosis(Shaoyin syndrome),which can improve the symptoms of neck and shoulder pain, warm hands and feet and other symptoms in patients with radiculopathy of cervical spondylosis(Shaoyin syndrome),and there is no adverse reaction or toxic side effects during the treatment.2.Through network pharmacology and molecular docking technology,it was concluded that Fuzi decoction may relieve the inflammatory reaction of cervical disc by affecting the pathway of NF-κB and TNF signaling, and thus has a better clinical effect on cervical spondylosis.
