Objective:The effect of Shenshan medicated diet on cancer-related fatigue was observed through clinical trials,and the clinical efficacy of Shenshan medicated diet was clarified.The method of network pharmacology was used to preliminarily explore its mechanism of action,which provided a reference idea for the treatment of cancer-related fatigue by traditional Chinese medicine.Methods:1.From December 2020 to September 2022,60 cancer patients with cancer-related fatigue were hospitalized in the Department of Oncology and Hematology of the Second Affiliated Hospital of Hunan University of Chinese Medicine.They were randomized to control and treatment groups.The control group was treated with conventional treatment,and the treatment group was treated with conventional treatment combined with Shenshan medicated diet orally.By observing the changes of The Revised Piper Fatigue Scale,MFI-20 score,TCM symptom scale and other scores before treatment and after 4 weeks of treatment,the clinical efficacy of Shenshan medicated diet was clarified.2.Through TCMSP,Uniport and other databases,the active ingredients and targets of 6 flavors of medicinal herbs contained in Shenshan medicated diet were retrieved.Gene Cards database searches were used to obtain targets for cancer-related fatigue.After obtaining the intersection targets of drugs and diseases through Venny_2.1,the String platform was used to construct a protein-protein interaction(PPI)network and screen out important targets.Cytosape3.9.1software was used to map the "drug-active ingredient-intersection target-gene" network.The DAVID database was used to perform GO(Gene ontology)analysis and KEGG(Kyoto encyclopedia of genes and genomes)enrichment analysis of intersecting targets.Results:1.The score of The Revised Piper Fatigue Scale showed that there was no significant difference in pretreatment score between the two groups(P>0.0.5).After treatment,the score of the treatment group decreased significantly compared with the pre-treatment score(P<0.05).Although the post-treatment score of the control group decreased compared with that before treatment,the treatment group decreased more significantly than that of the treatment group.2.The results of MFI-20 score on the multidimensional fatigue scale showed that there was no significant difference between the two before treatment(P>0.05).The post-treatment scores of the two groups were statistically significant compared with the pre-treatment scores(P<0.05).Among them,the post-treatment score reduction was more significant in the observation group.3.The results of TCM Syndrome Scale showed that there was no significant difference between the pre-treatment group and the control group(P>0.05).After treatment,the score of the treatment group decreased from before treatment(P<0.05),and the score of the control group decreased compared with before treatment(P<0.05).However,after treatment,the decline in scores in the treatment group was more pronounced.4.The regulatory network of Shenshan medicinal diet for the treatment of cancer-related fatigue includes 6 flavors of medicinal herbs,114 active chemical ingredients,and 149 drug-disease intersection targets.Its core components are mainly quercetin,kaempferol,etc.and the core targets are mainly AKT1,TP53,TNF,EGFR,IL-6.KEGG enrichment analysis of TNF signaling pathway,IL-17 signaling pathway,apoptosis signaling pathway,proteoglycan in cancer,pathway of cancer.etc.Conclusion:Shenshan medicated diet can alleviate the fatigue degree of patients with cancer-related fatigue and improve the quality of life of patients.Its mechanism of action in improving cancer-related fatigue may lie in inhibiting the secretion of inflammatory cytokines,regulating immune function,inhibiting tumor cell proliferation,and promoting its apoptosis.
