Clinical Observation On The Treatment Of Chronic Eczema Of Blood Deficiency And Wind Dryness Type With Tiger-shaped Copper-stone Scraping Therap

Objective: To observe the clinical effect of Hufu Tongbian scraping on chronic eczema patients with blood deficiency and wind dryness.Methods: 81 patients who suffer blood-deficiency and wind-dryness chronic eczema were selected from January 2022 to December 2022.All of them meets the selection criteria and are patients who have visited the outpatient department of Hunan Hospital of Integrated Traditional Chinese and Western Medicine to see a doctor.Based on the random number table method with the ratio of 1:1:1,81 patients were randomly assigned into 3 groups,Hufu Tongbian Scraping group,Hufu Tongbian Scraping combined with Zinc oxide ointment group and Zinc oxide ointment group.Patients in Hufu Tongbian Scraping group were given scraping once a week for4 weeks.Patients in Hufu Tongbian scraping combined with Zinc oxide ointment group were given scraping once a week and external Zinc oxide ointment twice a day,once in the morning and once in the evening,for 4 weeks.Patients in the Zinc oxide ointment group were given external Zinc oxide ointment twice a day,once in the morning and once in the evening,for 4weeks.The date of lesion severity,lesion area,itching degree,lesion area and severity index(EASI)score,Dermatological life Quality Index(DLQI)score and Patient Oriented Eczema Measure(POEM)was recorded before any treatment,2 weeks later and 4 weeks later respectively.4 weeks after the end of the treatment,the recurrence rate was collected and evaluated.Results:(1)There are no statistically significant differences(P>0.05)in gender,age,course of disease,skin lesion severity score,skin lesion area score,pruritus degree score,skin lesion area and severity index(EASI)score,skin disease life Quality index(DLQI)score and Patient Oriented Eczema Measure(POEM)before treatment among the three groups.Therefore,the three different treatments are comparable.(2)Comparison of skin lesion severity score and DQLI score among the three groups: the changes of skin lesion severity score and DQLI score before treatment,2 weeks and 4 weeks of treatment were statistically different among the three groups(P<0.05),and the skin lesion severity score and DQLI score of the three groups showed a decreasing trend over time;Compared with Zinc oxide ointment group,the skin lesion severity score and DQLI score of Hufu Tongbian scraping combined with Zinc oxide ointment group decreased significantly after2 and 4 weeks of treatment,and the difference was statistically significant(P<0.05).Compared with Zinc oxide ointment group,the skin severity score and DQLI score of Hufu Tongbian scraping group after 2 weeks of treatment had no difference(P>0.05),but the skin severity score and DQLI score of Hufu Tongbian scraping group after 4 weeks of treatment decreased significantly,the difference was statistically significant(P<0.05).Compared with Hufu Tongbian scraping group,Hufu Tongbian scraping combined with Zinc oxide ointment group,the skin lesion severity score and DQLI score decreased significantly after 2 weeks of treatment,the difference was statistically significant(P<0.05),but there was no statistically significant difference between the skin lesion severity score and DQLI score at 4 weeks(P>0.05).(3)Comparison of lesion area score,EASI score and itch degree score among the three groups:the changes of lesion area score,EASI score and itch degree score of the three groups were statistically different before treatment,2 weeks and 4 weeks after treatment(P<0.05),and the lesion area score,EASI score and itch degree of the three groups all showed a decreasing trend over time.Compared with Zinc oxide ointment group,the skin area score,EASI score and itch score of Hufu Tongbian scraping combined with Zinc oxide ointment group decreased significantly after 2 and 4 weeks of treatment,and the difference was statistically significant(P<0.05).Compared with Zinc oxide ointment group,the skin area score,EASI score and itching score of Hufu Tongbian scraping group decreased significantly after 2 and 4 weeks of treatment,and the difference was statistically significant(P<0.05).Compared with Hufu Tongbian scraping group,there were no differences in skin area score,EASI score and itching score in Hufu Tongbian scraping combined with Zinc oxide ointment group after 2 and 4 weeks of treatment(P>0.05).(4)Comparison of POEM scores among the three groups: There were statistical differences in POEM scores among the three groups before treatment,2 weeks and 4 weeks after treatment(P<0.05),and the POEM scores of the three groups showed a decreasing trend over time;At 2and 4 weeks of treatment,the POEM score of Hufu Tongbian scraping group was significantly decreased compared with the other two groups,with statistical significance(P<0.05);Compared with Zinc oxide ointment group,the POEM score of Hufu Tongbian scraping combined with Zinc oxide ointment group was significantly decreased,and the difference was statistically significant(P<0.05).(5)Efficacy evaluation: After the 4-week treatment,Hufu Tongbian scraping group has a total effective rate of 92.59%;Hufu Tongbian scraping combined with Zinc oxide ointment group has a total effective rate of 88.89%;Zinc oxide ointment group is has a total effective rate of 55.56%.These differences of the total effective rate among these three groups are statistically significant(P<0.05).There is no statistically significant difference between the effective rate between Hufu Tongbian scraping group and Hufu Tongbian scraping combined with Zinc oxide ointment group,and both of them have better effective rate than Zinc oxide ointment group(P<0.05).(6)Recurrence: After the 4-week treatment,Hufu Tongbian Scraping group has 8.00%recurrence rate;Hufu Tongbian Scraping combined with Zinc oxide ointment group has 12.50%recurrence rate;Zinc oxide ointment group has 46.67% recurrence rate.Conclusion: Compared with combined therapy,Hufu Tongbian scraping has the same curative effect in short-term comprehensive treatment of skin lesions and relieving skin pruritus,and is better than Zinc oxide ointment,which can improve patients’ quality of life in multiple aspects.The total clinical effective rate of Hufu Tongbian scraping group was comparable to that of combined therapy,and the efficacy of both groups was better than that of Zinc oxide ointment therapy.
