| Objectivesi. A research of the literature and clinical, initially established deficiency syndrome factors of IMN.ii. Through animal experiments, check efficacy of BuQi JianPi YiShen formula of C-BSA in rats.MethodsThe study contains three parts,that are research of IMN literature,syndrome elements characteristic of research,and experimental studyi. A research of the TCM literature about IMNThis study is reference to the systematic review. A database is establised, to analyze common symptoms, tongue, pulse, TCM syndrome, syndrome elements and medication treamen of IMN then made questionnaire. The data come from realted documents from January 1990 to December 2013, involving Chinese medicine, integrative medicine research and clinical literature about IMN treatment.ii. Clinical Research of the characteristics of deficiency syndrome elementsCollect outpatient and inpatient with IMN informations who met the inclusion criteria and to establish a database, including general informations. Study of the disease syndromes distribution of elements by analyzing clinical symptoms, tongue, pulse data.iii. Experimental study of treatment using method of BuQi JianPi YiShen to the MN ratsThe 48 healthy male Wistar rats were randomly divided into normal group (10 rats) and the model group (38 rats). Referring improved Border method, C-BSA rat model is established. After modeling, the model rats were randomly divided into three groups, model comparing group, traditional medicine treatment group (BuQi JianPi YiShen formula) and the comparison group (irbesartan). Generally of rats in each group were observed. Check 24hU-Pr of the rats once every two weeks before and after treatment. After 4 weeks, the rats were sacrificed; measure each group some chemical indicators. And take the kidney tissue, line ordinary HE stain and immunofluorescence observed between the groups of renal pathology. Application of SPSS 20.0 statistical software for data processing.Resultsi. A research of the TCM literature about IMN.included in the literature 135.a) Documents describe symptoms occur 56,statistical single symptoms 102,20 kinds of single Tongue, a single pulse 13 kinds. The most common symptoms of edema, fatigue, anorexia (>50%).b) Documents describe syndromes occur 86,92 describe syndromes, but frequency of occurrence≥ 10% of the syndrome is only 8. Cover syndrome elements 23, followed by five former kidney, qi deficiency, blood stasis, wet, spleen (>70%).c) Documents describe medicines occur 85,174 kinds of traditional Chinese medicine (regardless of origin and processing methods) involved; among the top five followed by medicine Astragalus, Atractylodes, Angelica, Poria and dangshen.ii. Clinical research of Deficiency syndrome elements included 91 patients, male to female ratio of 2.1:1, the age of 53.2 years ± 15.65 years. Most is within stage Ⅰ～Ⅱ.a) The clinical symptoms of IMN is hidden, before five symptoms followed by urine bubble, malaise, fatigue, edema, flushing, and backache petrified.b) Studies have shown that among 11 deficiency syndrome factors about IMN, top 5 are kidney,qi deficiency, body fluid deficiency, yin deficiency, blood deficiency (>33%); Syndrome factors of location are kidney, spleen, heart and lung. Kidney is much higher than other organs factor.iii. Result of experimental study treatment C-BSA rats by BuQi JianPi YiShen formulaa) At mental preparation stage, model group rats are in poor state, less activity, less amount of food and water, less shiny hair and unresponsive. After treatment, the model comparison rats gradually scrotal edema on the basis of the above symptoms, some have loose stools phenomenon. Chineses medicine treatment group and western medicine comparison group, the symptoms gradually improved to normal.b) Model group rats 24hU-Pr were all increased, compared to the normal group were significantly different (P<0.01), prompt successful model. After two weeks and four weeks of medication, treatment group and comparison group 24hU-Pr were significantly lower than the model group (P<0.01), but there was no significant difference between treatment group and the comparison group when medication getting longer(P> 0.05).c) The total cholesterol (CHO) level of each model group was significantly different than the normal group (P<0.05); but no significant difference between the model control group, the treatment group, and the comparison group CHO level. The blood urea nitrogen levels of treatment group and comparison group compared with the model group and the normal group were significantly different (P<0.05). There was no statistically significant among other indicators.d) Histopathological observation:HE staining under the model group and kidney tissue of rats administered group showed slightly increased glomerular volume, increasing the number of cells, including treatment group compared with the model group to a certain extent reduced. Observe the model group by direct immunofluorescence, the IgG along the glomerular capillary loops showed granular sedimentary discontinuity, the fluorescence intensity of Chinese medicine treatment group compared with model control group, western medicine group is weak, normal group IgG was negative.Conclusioni. Literature research suggest that the syndrome elements of IMN is mainly related to deficiency syndrom, and especially for kidney, spleen and qi deficiency.ii. Clinical study on IMN deficiency syndrome factors shows that kidney, qi deficiency is the most common syndrome elements of IMN. Location of based syndrome elements are mainly in kidney and spleen.iii. Through the experimental study on the treatment on C-BSA rats, draws the following conclusiona) C-BSA rat model was established successfully.b) Buqi Jianpi Yishen Formula can significantly reduce IgG in glomerular deposition of C-BSA rat model, and better than the control group.c) Buqi Jianpi Yishen Formula can reduce proteinuria in rats with C-BSA, curative effect and the western medicine comparison group.d) Buqi Jianpi Yishen Formula can reduce the level of BUN.
