Clinical Observation On The Treatment Of Idiopathic Membranous Nephropathy With Spleen-kidney Deficiency And Dampness-heat Type By Jianpi Yishen Dehumidification Decoction

Objective:To observe the clinical efficacy of Jianpi Yishen Dehumi dification decoc-tion in the treatment of idiopathic membranous nephro pathy(IMN)with spleen-kidney deficiency and dampness-heat type,hop ing to provide some reference for the clinical treatment of IMN with T raditional Chinese medici-ne.Method:Sixty patients with IMN who met the inclusion criteria were selected and randomly divided into 30 cases each in the treatment and control groups,and both groups received basic treatment(tacrolimus and general symptomatic treatment).The control group was additionally given nephritis kangfu tablets(0.3g×5 tablets/time,3 times a day,orally)in addition to basic treatment.In the treatment group,Jianpi Yishen Dehumidification decoction was added to the basic treatment.Take 12 weeks as a course of treatment.Record and compare changes in the Traditional Chinese Medicine(TCM)syndrome scores,24-hour urinary total protein(24h-UTP),albumin(ALB),cholesterol(CHOL),triglyceride(TG),serum creatinine(Scr),blood urea nitrogen(BUN),M-type phospholipase A2 receptor(PLA2R)before treatment,after 4 weeks of treatment and after the end of treatment in both groups,and,perform statistical analysis to evaluate the clinical efficacy of Jianpi Yishen Dehumidification decoction in the treatment of IMN.Results:A total of 60 patients were included in this study,30 patients in the treatment group and 30 patients in the control group,of which 1 patient in the control group fell off during the treatment process,In fact,a total of 59 patients completed this study,including 30 in the treatment group and 29 in the control group.(1)Clinical efficacy:After 12 weeks of treatment,the effective rate of the treatment group was 83.33% and that of the control group was 65.52%.Compared with the two groups,the effective rate of the treatment group was significantly higher than that of the control group(P<0.05).(2)Laboratory indicators:After 12 weeks of treatment,the 24h-UTP and ALB of both groups improved compared with those before treatment(P<0.05),and the improvement in the treatment group was slightly better than that in the control group.CHOL and TG in both groups improved significantly compared with those before treatment(P<0.01),and the improvement in the treatment group was significantly better than that in the control group(P<0.05).(3)TCM syndrome scores:After 12 weeks of treatment,the TCM syndrome score of the both groups were significantly reduced compared with those before treatment(P<0.01).The degree of reduction in the treatment group was significantly better than that in the control group(P<0.05).Conclusions:(1)Jianpi Yishen Dehumidification decoction can effectively improve the clinical symptoms in patients with idiopathic membranous nephropathy(IMN)with spleen-kidney deficiency and dampness-heat type,such as swelling of the face and limbs,fatigue and weakness,waist and knee pain,urination yellow red acerbity pain.(2)Jianpi Yishen Dehumidification decoction can effectively improve urine protein,serum albumin,blood lipids and other related indexes in patients with idiopathic membranous nephropathy(IMN)with spleen-kidney deficiency and dampness-heat type,delaying the progression of the disease.
