| [Background]Lung cancer has become the disease which has the highest morbidity and mortality rate in the world. The number of new patients has increased about 18 million in 2012, including 15.9 million of death. The new presenting cases of lung cancer patient were about 13% of the entire amount, and the lung cancer induced death were about 18% of the entire amount of death. In our country, lung cancer has also the most morbidity rate and mortality rate. There were about 6.5 million new lung cancer cases including 5.2 million of death. There may not have any visible symptom in the early stage, and the percentage of diagnosed cases was only 15%. There were about 80% of lung cancer patients were diagnosed at either locally advanced stage or having distant metastasis so that they had passed the best time to receive treatments, and the recovery was not ideal1. Even the technologies of multidisciplinary treatment for lung cancer has greatly improved, the 5 year survival-rate is only 10%-15.In addition, most of the treatments have the side effects, which could become an overload to the body. NCCN manual takes both of the extension of life time and decreasing of the size of tumor as the evaluation index of treatment effectiveness, but it hasn’t stand on the side of patients. Solving how to extending the life span while improving the quality of life has become to our goal.Not all kinds of cancers can be healed by the treatment methods of Modern medicine. The advanced stage of cancer is still meaning extremely painful, exhaustion, and death. The advanced cancer patients are usually weak, loss of self-care ability, and having complications such as cancer pain, thoracic and/or ascetic fluid, fatigue, problem of eating and breathing, and etc., even combined with cachexia, and bad quality of life by the cause of the tumor consumption and long-term repeated treatments. Base on the research2, about 76.4% of relatives and 77.1% of patients willing to have euthanasia when the process of the disease is death approaching and irreversible. So they had relieved painful and kept their dignity; recently in our country, there are about 50% or more3 of the fund which has been used on extending the death approached painful process of advanced cancer patients. This cannot make any improvement for the treatments of cancer, it cannot improve the quality of life for the advanced cancer patients to provide them a comfort and pleasant feelings in their last period of time, and it also goes against to the effectiveness usage of the limited medical resources. The cancer patients have suffering on both body and emotional level. Our hospice care has not yet developed completely. How to reduce the symptoms and improve the quality of life for the advanced cancer patients have become the challenge that must be figured out soon. Based on those, we have proposed a new treatment for cancer-- Green Synthesis Cancer Therapy. This therapy consists the points from both traditional Chinese medicine and western medicine, and it involves a low harmful, non-toxic non-side effects, and safety methods to heal cancer. Chinese herb therapies are very professional on cancer treatments based on its abundant theories and clinical experiences. Chinese herb therapies are guided by the theories; the treatments are based on syndrome differentiation for the principle; the processes involve individualized treatments; it is starting with the symptom reducing and life quality improvement; the mechanism of the treatments is to trigger the immune system to fight against the cancer tissue to consequently improving the toxicity of the chemical treatments and side effects.<NCCN Guide of Clinical Practice Oncology> was established by National Comprehensive Cancer Network (NCCN). It is the most widely used clinical practice guide for cancer treatments nationwide even global wide. But it has not yet mentioned anything about the effects and research results from involving Chinese herb therapies. How to give play to the advantages of the Chinese herb therapies, and how to build a mode of the "green synthesis therapy" for the cancer patients in China have become to an issue for now.[Objective]:1. Discuss the effectiveness of simplex usage of Chinese herb therapies on the advanced lung cancer patients; and provide the clinical evidences about using Chinese herb therapy as a method of hospice care by analyzing the supporting methods and the situation of symptom relief in the last six months of life to the advanced lung cancer patients.2. According to the retrospective study on the cases, discuss the effectiveness of the "green synthesis therapy" to advanced lung cancer patients; analysis the effectiveness of combination of traditional Chinese medicine and western medicine on the life time of advanced lung cancer patients; and provide the clinical evidences about using "green synthesis therapy" to the advanced lung cancer patients.3.Search the way to give play to the Chinese herb therapies, build a best treatment mode which will be the most suitable to our lung cancer patients, and provide references for the clinical treatments.[Methods]:The study involved 334 of death cases of the lung cancer in-patients at the Oncology Department of East Hospital from October,2009 to November,2014. All treatment methods were collected which also included the treatments given in the last six months of life. The treatment methods were organized, analyzed, and compared based on the effectiveness to the life time. The supporting methods which were given within the last six months of life were analyzed as well, and the factors that were affected on those supporting methods were discussed. The methods to give hospice cares by using Chinese herb therapies to the advanced lung cancer patients were also evaluated.[Result]:1.Analysis of Demography and Oncology related factors:ratio of male and female was about 2:1; it included 68.3% of senior patients, and most of the seniors were around age 70-79, which were about 39.5% of the entire studied cases. Most of the male patients suffered from either the small cell lung cancer or squamous carcinoma; most of the female patients suffered from the non-smoking glandular lung cancer; about 87.4% of the entire studied cases were diagnosed at middle or advanced stage.2.Comparison of different treatment modes that were effected on the life time:for advanced small cell lung cancer, the combination of traditional treatments with Chinese herb therapy could extend the life time for advanced small cell lung cancer patients (P<0.05); the simplex Chinese herb treatment could not improve the life time; for non-small cell lung cancer, both traditional and synthesis treatments which combined with Chinese herb therapy could extend life time for the advanced non-small cell lung cancer patients(P<0.05); but there was no statistical differences between the two (P>0.05); the targeted treatment which combined with Chinese herb therapy could not improve the life time for the squamous cell lung carcinoma patients(P>0.05); the synthesis treatment that combined with Chinese herb therapy was the most effectiveness treatment mode for glandular lung cancer, using the traditional treatment only was the second effectiveness treatment mode(P<0.05); and the targeted treatment that combined with Chinese herb therapy could extend the life time for the specific type of lung cancer which was unknown still(P<0.05).3.Discussion of the effectiveness of the "green synthesis therapy" to the life time:in the study, the comparison of the "green synthesis therapy" and simplex western treatment therapy can show the obvious differences on life time improvement.4.COX Analysis of life time:smoking, family history, surgery, chemical treatment, and radiology treatment were the factors which affected the life time. Smoking history was a risk factor of life time (B-0.339, RR-1.403); in contrast, family history (B=-0.793, RR-0.367), surgery (B=-1.002, RR=0.367), radiology treatment (B=-0.363, RR=0.696), and chemical treatment (B=-0.751, RR=0.472) were the advantaged factors for life time.5.Decision of choosing a treatment method for patients in their last six months of life:there were only 47.4% of patients picked the Chinese herb combination supporting treatment; about 40.0% of patients were still preferred using the attacking treatments such as radiology and chemical treatments; and about 12.5% of patients would like to use targeted treatment.6.The effective factors by different supporting treatments which were used during the last six months of life of the advanced lung cancer patients:patients who were receiving the simplex Chinese herb synthesis supporting treatments had better situation(P<0.05) than the patients who were using the combination of traditional Chinese medicine and western medicine treatment(P>0.05):it involved lower rate of using blood transfusion, lower frequency of using blood pressure boost injections, pain relief pills, hemostyptic, theophylline and steroid drugs, and the short usage periods of theophylline, sedative, and steroid drugs, lower possibility of getting hydrops, and shorter length of stay in the hospital.7. Analysis of the cases that had longer life time:the treatments that could extend the life time for advanced lung cancer patients were selective to different types of lung cancer:glandular lung cancer and squamous carcinoma were benefited more than the small cell lung cancer; the selectiveness between glandular and squamous carcinoma lung cancer were not obvious, but for the 2-year living condition, glandular lung cancer patients were benefited more; the differences on sex were not obvious, too.[Conclusion]:1.Earlier and energetic synthesis treatment could effectively improve the life time for the advanced lung cancer patients;2. In the study, comparing the "green synthesis therapy" guided combination of traditional Chinese medicine and western medicine treatment with the simplex western medicine treatment, there were no obvious improvement to the life time.3.There were only about 47.4% of patients who used the simplex Chinese herb treatment during the advanced stage of the lung cancer, most of the patients used attacking treatments as the first choice.4.The simplex Chinese herb treatment could obviously reduce the symptoms of advanced lung cancer patients, and it can also improve the quality of life during the advanced stage of the cancer. Thus it could be one of the hospice care treatments.
