Purpose:The purpose of this study is to analyse patients who suffered the Diabetic foot level0without ulcer symptom, through constructing the differentiation table of Diabetic foot level0and creating new methods of Syndrome Element Differentiation, so as to find out the regular treatment that is about the symptoms of Diabetic foot level0, and to provide the significant theoretical basis of diagnosing and preventing the Diabetic foot symptoms in early stage.Design/methodology/approach:This paper firstly analysed the research history of Diabetic foot by reviewing the previous literature of Chinese Medical such as diabetes, ulcer, gangrene, and blood-arthralgia.By summarising the findings of previous research, author summarised the limitation of differentiation table and the method of Syndrome Element Differentiation. Based on the aim of achieving the clinical utility as well as dialectical thinking of Chinese Medicine, information was gathered from related medical books, teaching materials of universities and colleges of Chinese Medicine, international and industrial standards. Then, the database of basic symptoms was adjusted (short for database A), whilst the database of basic symptoms code of Diabetic foot level0was constructed either (short for database B).In addition, this paper utilised the epidemiological method to clinically analyse the Diabetic foot level0. Specifically,256patients who matched the symptoms of Diabetic foot level0have been selected as samples; meanwhile, the patients’comprehensive information of disease has been collected and filled in the new table. Then, the related code in database B of each sample’s symptoms has been found out, whilst the repeated codes were combined. After that, the frequency and weighted integral were calculated. Based on the basic syndrome element code, the entire name of basic syndrome elements were selected in database A; moreover,. Semi-quantitative Method of Mathematical Statistics and Small Sample Method were adopted to set up the best trade-off threshold, and the gathered syndrome elements have been analysed comprehensively. Finally, the results were determined with the combining of dialectical thinking.At last, the regularity of the symptoms of Diabetic foot level0for the256samples has been analysed by using the methods of frequency, weighted integral, syndrome element and experts’experience, based on the results which were carried out by experts.Findings:·Through reviewing previous literature, the name, pathogen and pathogenesis of Diabetic foot level0were summarised, which is valuable to prevent and treat this disease.·This paper carried out a new approach to construct the differentiation table, which advocated that the table should contain the symptoms and four ways of diagnosis. The former could make sure that the treatment has its specific target and prevent bias, whilst the latter could promise the integrity of clinical information, which is helpful to think dialectically and to develop the clinical work.·A new database of disease symptoms has been constructed. The basic syndrome element of symptoms were not taken from the specific samples; however, it came from the medical books, teaching materials, international and industrial standards, which has thousands years’ history in China. This kind of database is not bounded by the difference of disease, and it is handled by the modern Chinese Medicine, which has a strong potential of further application.·The disease name determination has been improved. By calculating the frequency and weighted integral, as well as combining the clinical thinking, the disease name could be determined. This is objective, reliable, and it does not loss the pith of Chinese medical dialectic.· Summarise the symptom regulation of the diabetic foot level0:First, disease location mostly focuses on the spleen, kidney and liver. To be specific, the spleen had194cases and took up75.78%. The kidney had83cases and accounted for34.42%. The liver had60cases and held23.44%.Second, the nature of disease is mainly about phlegm and dampness (account for94.14%), and most of them accompany with blood stasis. Generally speaking, patients have the symptom of the deficiency of Qi and the blood, coupled with the deficiency of Yin and Yang. In addition, Yang deficiency is the most common, which237persons had and took up92.58%.199persons had blood deficiency and accounted for77.73%.125persons had Qi deficiency and occupied48.83%. Therefore, this study on the nature of disease combines deficiency and excess, which is mainly about Yang-Qi deficiency and blood deficiency, as well as phlegm, dampness and stasis.Third, the main characteristic of symptoms is Spleen Yang deficiency and Spleen-kidney Yang deficiency. Meanwhile, each main symptom also mixes with several minor symptoms, and Spleen Yang deficiency appears the most frequent, which included125samples (account for49.22%), whilst68samples suffered Spleen-kidney Yang deficiency (account for26.56%).Conclusion:Diabetic foot level0not only comprises the basic pathogenesis of Yin deficiency and dryness heat, but also concentrates on the spleen, kidney and liver. The nature of it performs Yang-Qi deficiency and blood deficiency, combing with phlegm, dampness and stasis. Common symptoms are mainly Spleen Yang deficiency and Spleen-kidney Yang deficiency. This characteristic could provide important information for diabetic foot’s early diagnosis and prevention as well as treatment.
