| Objective:Through clinical observation, score the quality of life, TCM syndrome and urine volume calculation,analyze the effectiveness and safety of the treatment of patients with malignant ascites treated with Wenyanglishui Formula which was based on literature research,it could provide new ideas and methods, explore the impact on the cytokine regulation, provide clinical service for the application of TCM for external use, so as to conduct the further clinical promotion.Methods:Using prospective,single-center study, propose diagnostic criteria, inclusion criteria and exclusion criteria.All 60 observed cases were choosed from January 2013 to August 2015, oncology which were conformed to the standard of patients with malignant ascites.They were divided into 2 groups, the treatment group and the control group.The specific method of the external use of the treatment group was:Took Shenque Xue to paste the central point of the adhesive point washed and dry, and made the drug powder into a paste with warm water and mixed thoroughly, placed 25 × 31cm dressings central area, gauzed expand coverage, drug ranged from about 8×10 cm, drugs thickness about 2mm, around the tape securely gauze affixed to the abdomen, every 4-6 hours to remove the dressings and drugs for 2 weeks.The control group was treated with diuretic therapy with 20mg of torasemide per day for 2 weeks also.Recorded the changes of the two groups involving abdomen circumference and 24h urine output, quality of life and TCM syndrome score and skin reactions, inspected IL-4 and IFN-y, carried statistical treatment at last.Results:Analyze the data, blood IL-4 and IFN-yof the treatment group were statistically significant than the control group; the fluid largest diameter change, abdominal circumference change and the 24-hours urine volume change of the two groups of patients after treatment than before treatment of ascites, were statistically different, which revealed the two groups of treatment could improve ascites while the treatment group were more effective than the control group. According to the clinical symptoms of traditional Chinese medicine integral evaluation, the abdominal distension and urinate short, abdominal pain of the two groups were improved after treatment than before treatment, which was statistical significant, and the treatment group was statistically significant than the control group.According to security, there was no significant clinical changes in routine blood, liver and kidney function, electrocardiogram.Some patients had a mild skin reaction, which disappear after the drugs was stopped, considering no factors about the security.Conclusion:The external use of the Chinese medicine of Wenyang lishui Formula with diuretic therapy could improve patients with fluid situation, made abdominal circumference and 24-hours urine amount improved, which was obvious than with a simple application of diuretic therapy. In addition, by measuring blood levels of IFN-γand IL-4, judged Th1/Th2 state, reflected the mechanism of the Fomula treated with ascites was related to the regulation of cytokines, by adjusting the Thl/Th2 balance, regulate the body’s immune function.At the same time,the Fomula could relieve patients’clinical symptoms and improve the quality of life.From the perspective of traditional Chinese medicine, this formula could make the sanjiao gasification,kidney governing excretion and the descending function of lung qi normal through warming and transporting yang qi of spleen and middle energizer,so as to achieve water passage tuning, harmony between qi and blood, Zang-fu function unobstructed, finally water and distension gone.
