Background:Colorectal cancer is a frequent tumor in the world, and its incidence is increasing in recent years in China. Its therapy include surgery, chemstry, radiotherapy, biotherapy and so on, but its effect is not satisfactory. Traditional Chinese Medicine (TCM) plays an important role in treatment by improving the life qulity and prolonging life span. In the long term of clinic, we found that cancer treatment can be divided into four periods basing its characters. There are preoperation period, adjuvant therapy period, follow-up period and palliative therapy period. The key point of TCM teatment is syndrome-differention which is the foundation and precondition. Study of the syndrome of TCM includes syndrome standardizing and essence research. In the past few years, experts focus on restrospective study with subjectivity, so that the standard was not unified. On the other hand, indexes involving many aspects are not repeatable. The two reasons inhibit the progress of the TCM syndrome study. TCM Puts emphasis on the whole status of body which is the combined results of specified proteins appearing in different time or space.Proteomes research reflects the all proteins as a integrated leval of the disease. So proteomes is considered to be able to interpret the nature of syndrome.Objectives:Establishing the standard of different syndrome in colorectal cancer patients with adjuvant therapy by using mathematical statistics. To explore the essence of TCM syndrome of colorectal cancer in adjuvant therapy period by analyzing different expression of serum proteome of 32 cases.Methods:Investigates 179 cases by cross section prospective clinical epidemiology, and classifies TCM syndrome of colorectal cancer with main clinical symptoms of each one by Clustering and Factor analysis. Serum proteome of 32 cases of constitution of nonsymptomatic group, Agglomeration of Dampness and Heat group, Deficiency of liver-yin and kidney-yin group, and Spleen Deficiency and Dampness Retention group were separated by 2-D gel electrophoresis(2-DE). The gels were respectively image analyzed by DeCyder Image QuantTM V6.5 in each paired DIGE. MALDI-TOF-MS was used to provide sensitive and accurate mass spectral data for the unique proteins changed in abundance more than 1.2 folder up or down compared with which in constitution of nonsymptomatic group, and then clusterin protein is confirmed by Western blotting.Results:1.179 cases are classified by Clustering Analysis combined with the basic theory of TCM and our clinical experience into 4 syndromes as following:Spleen Deficiency and Dampness Retention, Deficiency of Qi and Blood, Deficiency of liver-yin and kidney-yin> Agglomeration of Dampness and Heat. The main clinical symptoms of each one were induced by Factor Analysis.2.11 proteins were identified in four group cases. They are albumin preproprotein, sulfated glycoprotein-2, hemopexin precursor, GDP dissociation inhibitor beta, zinc finger protein 416, apolipoprotein A-1 precursor, clusterin, CD5 molecule-like precursor, transferring, haptoglobin, vitamin D-binding protein. They are correlated to immunity, tumor, inflammation and energy metabolism.Conclusions:1. Using epidemiological study combined with mathematical statistics can improve standardization in TCM syndrome of colorectal cancer. It is significant to carry out large sample and multicenter epidemiological study to guide treatment in clinic.2. The different syndrome of palliativ therapy period in colorectal cancer could be attributed to the amount of proteins’ expression. Researeh technique of "Different syndromes with same disease" is the way to filt the specific object target of syndrome nature.
