Study On The Application Of Renal Shear Wave Velocity And BMP-7 And CTGF In The Diagnosis And Treatment Of Primary Nephrotic Syndrome In Children

Objective This study applies VTQ technology to detect the SWV of children with primary nephrotic syndrome(PNS) and healthy children, and detect the level of serum BMP 7 and CTGF. Then correlation analysis was made between BMP-7,CTGF and SWV,to explore the value of BMP-7,CTGF and renal SWV in the diagnosis and treatment of renal interstitial fibrosis in children with PNS.Methods 150 cases of children with primary nephrotic syndrome were divided into three groups according to the damages of tubulointerstitium,according to the pathological results of renipuncture.: mild,moderat,severe renal interstitium fibrosis groups.50 healthy children were selected as the control group. The level of serum BMP-7 and CTGF was detected and the virtual touch tissue quantification(VTQ) was used to detect renal SWV in four groups. Both the serum BMP-7,CTGF and the renal parenchyma SWV were compared between four groups. Then correlation analysis was made between BMP-7 、CTGF and renal SWV in children with primary nephrotic syndrome.Results The age,gender,BMI of the control group and PNS group had no statistical difference;Urine routine,BUN,CRE also had no statistical difference;however,24 h Ualb, blood albumin, T-CHD had statistical differences P<0.05.Serum BMP-7 level: the lowest was mild group, the middle one was moderat group,and the highest was severe group(P<0.05);serum CTGF level and renal SWV values: mild group was the highest, moderat group was middle one and the severe group was the lowest(P<0.05);Compared to the control group, the group of mild renal interstitium fibrosis had no statistical difference, but the other two groups had(P<0.05).The values also had statistical differences between PNS groups(P<0.05);The serun level of BMP-7 and renal SWV had significant negative correlation(r=-0.672, P<0.05);The serum level of BMP-7 and CTGF had significant negative correlation(r=-0.798, P<0.05);The serum level of CTGF and renal SWV had significant positive correlation(r= 0.702, P<0.05).Conclusions The serum BMP-7 level reduced with the increasing of degree of renal interstitial fibrosis, but the renal SWV and serum CTGF level increased with the degree of renal interstitial fibrosis elevating, and renal SWV may indirectly reflectd serum BMP-7 level. All of them had important application values in the diagnosis or treatment of Primary nephrotic syndrome.
