Doctoral postgraduate: Tang zhipeng Advisor:Prof. Xu xinmeiGastric ulcer(GU) is a multifactorial deep necrotic defect lesion involving the entire gastric mucosal depth and the muscularis mucosae, which is detrimental to health of human being. It has been proved that Jianzhongyuyangpian(JZYYP) is an effective anti-ulcer prescription which increases gastric ulcer healing rate and decreases relapse rate clinically. In order to investigate the influence of JZPPY on the quality of gastric ulcer healing and its therapeutic mechanisms, the following studies were carried out.1. Literature researchThe term, etiology, pathogenesis and treatment of GU have been studied and drawn some certain conclusions by traditional Chinese medicine(TCM). GU belongs to the fields of "epigastralgia", "acid regurgitation", 'Vomiting", and "sore". The research data show that the occurrence of GU is affected by emotional disturbance, invasion into the stomach of exopathgen, and impairment due to overstrain. GU is located in the stomach and related to the liver spleen closely. In the early stage of the disease, the location is in the Qi, and later in the meridians. The nature of GU mainly belongs to coexistence of deficiency and excess. Namely, the deficiency of spleen and stomach is a pathological basis and accompanied by damp heat, Oi stagnation, phlegm and blood stasis. At present, the main pattern of TCM and differentiation of diseases of modern medicine. Generally speaking, the effectiveness of clinical treatment is satisfactory. Meanwhile, the therapeutic mechanisms of Chinese medicinal herbs in treating GU have been widely investigated, the data indicate that Chinese medicinal herbs have the following functions: inhibiting acidand pepsin secretion, killing Helicobacter pylori(Hp), decreasing reactive oxygen species damage, increasing mucus secretion, increasing prostaglandin Ej concentration, and stimulating mucosal cell proliferation.In modem medicine , the etiology and pathogenesis of GU is due to the imbalance between aggressive factors and mucosal defensive factors. Gastric acid, Hp infection, and impaired gastric mucosa defensive function are the most important factors. Although therapies by histamine H2 receptor antagonists and proton pump inhibitors can bring about rapid healing of GU, discontinuation of these therapies may result in significant recurrence of ulceration. It is believed that the ulcer relapse is closely related to Hp infection and quality of ulcer healing(QOUH). QOUH may become a key factor involving in the process of Hp infection inducing ulcer relapse. Mucosa of healed GU displays histological abnormalities that are possibly the basis of ulcer relapse. Clinical and experimental data show that some gastroprotective agents can improve the QOUH of GU.2. Experimental studyResearch objectsThis study estimated the QOUH of acetic acid -induced gastric ulcer in rats treated with JZYYP and investigated the possible molecular biology, cell kinetics, growth factor, and cytokine aspects of therapeutic mechanisms.Materials and methods(1) Animal model and treatmentThirty male and thirty female Sprague-Dawley rats weighting 18O~220g were used .Ten of them served as intact controls, while the other rats were operated on. Three days after the establishment of acetic acid-induced gastric ulcer, the rats were divided into six groups: intact control, GU model , placebo, ranitidine, JZYYPI(small dose), and JZYYPH(large dose) groups. Rats of GU model were executed on day 3, while the rats of other groups were executed when treated for 14 days. Then, gastric tissues and serum ?samples were collected.(2)Quantitative histological assessmentThe specimens for histological examination were fixed with phosphate-buffered formaldehyde and embedded in paraffin. Serial sections,5um thick, were cut perpendicularly to the surface of the mucosa. Then, the sections were stained with HE, AB-PAS, VG, and SP immunohistochemistry methods.
