The new method of photodynamic therapy (PDT) combined with non-coherent light (red light) for port wine stains(PWS) was studied . The animal model were adult male chickens. The red light (600-700 nm) produced by halogen tungsten lamp irradiated the surface of chicken comb with different light dose after administration of the photosensitizer PsD-007 intravenously The change of the comb color were observed by gross observations, and the tissue of comb histologically to study the factors which are the light dose,the time for irradiation, the distance of irradiation and the dose of PsD?07 on the result of PDT. Then the method was used to treat the different kind of PWS clinically. The main results and conclusions are as follows: l.After injection of photosensitizer into vein ,the amount of photosensitizer was rich in vascular lumen of comb during the early time , later decreased , and increased gradually in the mesenchyme of comb 2.The comb color of the acute PDT-treated area were pink , purple , purplish blanching and blanching separately and renewed at 8 weeks . There was no apparent damage to the epidermis of comb . The comb had temporal reactions to the PDT with the irradiating at 30 mm after administration of the photosensitizer. The endothelial cells and erythrocytes in the capillaries of the acute PDT-treated combs were swelling and degeneration .The erythrocytes accumulated in vessel lumens and formed thrombus .The bigger vessels in the medial dermis were reopened. There were revascularization in 2 the superficial dermis. Finally, the vessel architecture of the comb were recovered. 3. The deeper vessel architecture were damaged selectively by increasing the dose of PsD-007 or the amount of the light dose ,or by shorting the distance of irradiation. 4. The red color of the lesions of PWS in the irradiated area became dark purple at 1 days , black purple at 3 days and 5 days ,and fading gradually in 7 days .Some of the lesions disappeared at 2 weeks. The skin of the purple lesion with proliferation , treated ,had thin crusts that would take off at 3 or 4 weeks after PDT . The red color of the PWS disappeared completely without scar after PDT. There were some pigments where the crust took off , which became normal color in 6 months after PDT. 5. Eighteen cases treated have excellent results after PDT one time . The red color of lesions disappeared completely, and the skin became normal in 11 cases. Most parts of the red color of lesions disappeared and became flat in 5 cases , and became thin in 2 cases. There were no recurrence in 3 to 36 month follow-up period. 6. It was demonstrated that PDT combined with non-coherent light (red light) could occlude the malformative vessels in PWS just after administration of exogenous photosensitizer with preserving the normal skin.
