This article created rat island flap I/R injury model ,used LM,TEM,SEM,FCM et al machine and two -sandwich ELISA,H-E et al method, studied a series of neutrophil activation in I/R flap injury and the modulation of dexamethasone .5 parts were included in this paper .㏕he model of rat flap I/R injury and the protection of different dexamethasone doses. 〢ctive neutrophil in I/R flap and the modulation of dexamethasone 〤hanged neutrophil hemorheology (adhesion and deformability) in I/R flap and protective mechanisms of dexamethasone?Unnormal PMN died process in I/R flap and the modulation of dexamethasone 〨eneral effects of local flap I/R injury and the protection of dexamethasone .We concluded that (1) dexamethasone could decreased flap I/R injury , dexamethasone should be used as early as possible when occurred I/R injury . (2)Active PMN has important role in I/R flap injury , Dex could decreased active PMN level. We firstly confirmed the microstructure of active PMN in I/R injury flap. (3)Increased PMN adhesion ability and decreased PMN deformability were important-6-reason of I/R flap ;Dex could restored PMN normal hemorheology. (4)UncontrolledPMN died process (PMN high level necrosis and low level apoptosis early in I/R injury and high level apoptosis later in I/R injury ) and unnormal macrophages .neutrophil in flap could not digested apoptic PMN ,many necrotic PMN could created a lots of poison materials. So injury flaps.Dex could modulate PMN died process .sustained normal PMN in flap .avoided delayed apoptotic PMN. Thus protect flap. Also we firstly testified this PMN aspects in I/R flap injury. (5)High level of TNF-a and low level IL-10 in serum were also the reason of flap I/R injury. Dex could modulate this imblance phenomenon .We first find this aspects in I/R flap injury. (6)Local I/R injury could make general effects, which showed white thrombosis in vasculer and lung, intestinal pathological changes .We also first find the intestinal changes in I/R flap injury. Dex could ameliorated this general effects .In summary , a series of PMN activation .such as active, hemorheology changes, unctolled died process, imblance of TNF-a to IL-10 were important reason in I/R flap injury. Dexamethasone could modulate all these aspects, so could protect flap from I/R injury. Simple one method, such as anti-free radicals could not achive good flap protective results.
