Objective To investigate the regulated efficacy of Tangshenkang which based on strengthening the spleen and kidney and activating the blood circulation,on renal hemodynamic changes in early diabetic nephropathy (DN),and the relationship with hitric oxide(NO),to study the mechanism involved in it and explore the theoretical foundation in early prevention and cure. Methods 100 patients with DN were divided into two groups by random,among the patients,test group(60) were treated by Tangshenkang, control group (40) were treated by Gliquidon,one month was a course of treatment. Results The clinical study showed that Tangshenkang had good therapeutic efficacy in DN,especially in early DN,could effectively improne the clinical symptoms,descended the blood sugar and blood lipid,improved the blood rheology,descended the microalbum in urine and improved the renal function.The experimental investigation also showed that Tangshenkang could descend the supernormal NO in the DN model rats obviously,as well as the blood sugar, glycosylated hemoglobin,blood lipid and microalbum in urine. Conclusion Increment of NO synthesis in early DN may be an important factor in the renal hemodynamic changes,Tangshenkang could improve the renal hemodynamic changes by desreasing the NO level, and then played an important role in the early prevention and cure of DN.
