Vioding dysfunctions are the common problems, with high incidence rate, complicated conditions and low cure rates for most patients, moreover the results are unsatisfacted for some sophisticated patients. The main methods we used nowadays for the dysfunctions are as following: behavioral practice, medicine and surgery. Electrical stimulating(ES), as a new method, was firstly studied in 1970s, and was proved to be one of the effective therapy. Electrical stimulating means to stimulate the organs directly, or stimulate their innervative nerves indirectly, using some proper electric current. This will change the functions of the lower urinary tract, including the bladder and the urethral, and finally change the urine storage and voiding. Nowadays ES gradually becomes one of the critical therapies for the dysfunctions of lower urinary tract symptoms(LUTS), especially for some special dysfunctions.ES for the LUTS is divided into four species according to the positon: 1. stimulate the central nerve directly. Theoretically this method would be most effective, but actually it is restricted for its side-effects, such as the current expansion and infection. 2. stimulate the sacral nerve root(SNR). This position can modulate the biological function of the central nervous, therefore to get good result of storage and voiding. Its main shortcoming is the strong request of the machines. 3. stimulate the periperal nerve, such as the pudendal nerve and the pelvic nerve. This method is effective in studies on animals, however, for the clinical patients, it's restricted because it's difficult to suspend the stimulator on the periperal nerve. 4. stimulate the target organ directly, such asto stimulate the detrusor for voiding, or to stimulate the sphincter for continence. It's easy to perform, with satisfacted effect. As for the articles, the most effective and practive methods are the electrical sacral nerve root stimulation(ESNRS) and the peripheral stimulation.So far there are still some shortcomings in ES, they are: 1. the definite effect on some special dysfunctions is not certain. 2. the best position for ES. 3. the proper parameters. 4. the probable mechanism..5. the possibility and the carameters for our own nation. 6. lack of practical machines.Object: aim at the needs to the dysfunctions of LUTS, we are to study the following: 1. to observe the effect of electrical pelvic floor stimulation(EPFS) on the bladder storage and the urethral continence of animals, and its probable mechanism as well, in order to provide the basis for clinical treatment. 2. based on the study of animals, to observe the effect of EPFS on the clinical patients. 3. as for the sophisticated dysfunctions of LUTS, we should use the ESNRS. So we are to observe the effect of ESNRS on the neurogenic bladder, and its probable mechanism as well. 4. based on the studies of animals, to observe the effect of ESNRS on the sophisticated clinical patients.Method: (1) to stimulate the goats with the self-maken stimulator, and to observe the urodynamic, biochemical and the structural changes between the stimulating group and control group, in order to detect the effect of EPFS on the storage and voiding. (2) to select the fitting patients for stimulated by the self-maken stimulator, therefore to detect the changes before and after stimulated. (3) to make the model of neurogenic bladder by traversing Ll-2, then stimulate the sacral nerve root by the self-maken stimulator, and to observe the effect of ESNRS on the urodynamic and apoptosis of neurogenic bladder. (4) to observe the effect of the percutaneous electrical sacral nerve root stimulation on the sophisticated dysfunctions of LUTS.The main results and conclusions were as follows:1. ES is one of the major methods for LUTS. By the experimental and the clinical studies of the PF and the SNR, we have got the curable effect on the LUTS, especially for some certain dysfunctions.2.
