Most patients with breast cancer are treated with combinedsUrgical theraPies. StUdying on the tUmor vessels of breastcancer and chemotheraPCotic effect, we try to find the characteristicof the tumor vessels in breast cancer, and confirm the effect ofneoadjuvant intraarterial chemotheraPy in the treatment of breastcancer.Part I. The Tumor Vessels of Breast Cancer onImaging Study and the Relationship with Tumor StagingObjective: To study the image characteristic of the tumorvessels in breast cancer by digital subtraction angiograPhy (DSA),and the relationship of the tumor vesseIs and tumor staging.Materials and Methods: Eighty-six female patients with breastcancer were in-patients(stage I 8 cases, stage IIA l9 cases, stageJIB 29 cases, stage IIIA 2l cases, and stage IIIB 9 cases). Therewere eight cases with tumor size less than 2cm, 56 cases with tumorsize betWeen 2cm to 5cm, and 22 cases with tumor size larger than5cm. After sUrgery, the numbers of lymph node and axillarymetastases were recorded. The percentage of axillny lymPhaticmetastases was calculated. The main observed images included theorigin of tumor suPply arteries and the characteristics of tumorvessels. Results: We fOund 67(77.9l%) cases with arterialabnormity and tumor vessels, in which 5l(76. l2%) abnormal vesselsoriginate from arteriae thoracica lateralis, l6(23.88%) from arteriaethoracica intema and arteriae thoracica lateralis. The main abnormityof allery and tumor vessels are terminal small artery increasing andtWisting irregUlarly. IrregUlar vessels dyeing can be seen in thelocation of breast tUmor in 54 patients. The outflow vein becamewide. According to TNM, the tumors were divided into three grades(<2cm, 2~5cm, and >5cm). The difference in breast cancer betWeenwith vessels dyeing and without vessels dyeing is very significant. Itindicated that tumor vessels have closely relationship with the tUmorsize. Pathological lymphatic metastases were fOund in 4l of 86patients with breast cancer (with tumor vessels 33 cases, withouttumor vessels 8 cases). Statistical result presents that the higher rateof lymPhatic metastases can take place in the case with tUmorvessels. Conclusions: Breast cancer is of multi-vessels tumor. Mostsupply arteries origin from arteriae thoracica lateralis. Tumor vesselsare closely related to the size of tuinor. When tllmor vessels present,lMhatic metastases will increase.Part n Pathological Assessment of Response toNeoadjuvant IntraarteriaI Chemotherapy in patients withBreast CancerObjective: Pathology review was used to confirm the effect ofneoaduvant intraarterial chemotheraPy in breast cancer. Infection oftumor vessels on the effect of intraarterial infusion chemotheraPywas assessed. Materials and Methods: The CEF chemotheraPy wastaken in 86 patients by intraarterial infusion chemotheraPy. Theinterval betWeen chemotheraPy and surgery was average 8 days(1~27 days). According the DSA images, we divided patients intotWo groups, the group with tumor vessels and the other grouPwithout tumor vessels. The effect of chemotheraPy in breast cancerwas classified as no effect (no change in tissue), effect (slight tissueresponse), obvious effect (middle tissue response), and specificeffect (high tissue response). Results: Pathological morphologicalchange was the different degree denaturalization of cancer cells instage I. In stage II, cancer cells degenerated further, evennecrosed. The numbers of cancer cells decreased. Cytoplasmicvacuolization was the most strildng change. In stage m, thenumbers of tUmor cells decreased markedly.
