| According to the latest achievement and tendency of the protective mechanism of the second window of ischemic preconditioning, the mechanism of apoptosis in cardiocyte and the effect of pharmacological preconditioning, the reperfusion model is set up to study the areas of myocardial infarction by N-BT staining method and to observe the ultrastructure changes of myocyte by optic and electron microscope. The level of NO, SOD and LPO were detected by biochemistry element methods, the protein expressions of HSP70, BCL-2 and Bax were detected by immunohistochemistry, the mRNA expression of BCL-2 and Bax were detected by TUNEL method, and the apoptosis rate and active oxygen were detected by flow cytometry.The results indicate that both apoptosis and necrosis are co-existent during the ischemia reperfusion injury. The second window of ischemic preconditioning can increase the activity of SOD, NO and the expression of HSP70, inhibit the production and release of Ros. At last, it can shrink the area of myoinfarction after reperfusion and relieve the injury in ultrastructure of myocyte. We also found, in farther research, the protection of second window of ischemic preconditioning can inhibit the myocyte apoptosis through increasing the expression of BCL-2and decreasing the expression of Bax and apoptosis rate. It confirms that the protection of second window of ischemic preconditioning alleviate the ischemia reperfusion injury. Wenxin Capsule, a Chinese medicine, based on the theory of warming yang and nourishing the heart and promote blood circulation and dessipating plegm, has protective effect in the ischemia reperfusion injury through the protection of secondwindow of ischemic preconditioning. That protection mechanism embodies the reduction of infarction area, myocyte ultrastructure injury and the inhibition of myocyte apoptosis. It is worthy of trying to find a new a pharmacological preconditioning medicine, to develop a new field of heart protection.
