Ulcerative colitis is a nonspecific chronic colitis.Theaffection mainly on the mucous and submucous layer of colitis.mainly showing in flammation and ulcer first invation rectum and distal colitis.Major sysptom ar.diarrhea.pus-bloody stool.abdominalgia andtenesmus.Course of diseasa is long,reapeated attack.Then incidence is lower than abroad in our country.Irecent years, thissickness is increasing.In 1987,it isreported that didn't reach 1000 cases, but from 1987 to 1993,have reached 7137 cases.In recent ten years, The doucument about IBD have overstepped 1000 pieces, case overstepped twenty thousand,among them,the case of US increase evident.This sickness can fall in any age,more in 20 to 30, male over female, In Europe and America UC is a common disease, morbility rate about 40-100/ten thousand,incidence 3-11.5/ten thousand.There is not accurate statistics report in the country, but the tendency is increasing obviously.Based on the onset features of ulcerative colitis (UC) in our country, this study analyses the development in the treatment of UC with TCM and Modern Medicine method, and put forward that TCM has many advantages in the treatment of UC. We sum up the experiences, select effective recipes and triturate the JieChangling (JCL) It shows significant effect in the primary clinical observation.In the study, 62 patients with UC selected were randomized divided into two groups, one is treated by JCL, and the other, the control grouOp, by SASP. We observe the changes of symptoms, intestinal mucous image under colonoscopy, the histopathologic morphologic changes of intravital microscopy, and investigate the CD subset of T cell, circulating immune complex (CIC), immunoglobulin Ig, NK cell's vitality, etc. The results suggest thatthe patients with UC has disorder of immune function, the CD3\ CD4+ cell count and the ratio of CD4+/ CDg+ increase abnormally, the CD8+ cell count decrease as well. These show the abnormal increasing of humoral immunoresponse. In the other hand, the IgG^ IgM and CIC in peripheral blood increase, it may cause the intestinal mucous deposition of CIC and activate the local allergy. The patient with UC also has a decreased vitality of NK cell contemporaneously, so its restraining function decrease, and the abnormal discretion of B cell happens, lots of CIC deposit on the intestinal mucosa, the immune system was activated and attack epithelium of intestinal mucosa it lead to the allergy.All of above is according with the images under colonoscopy such as superficial ulcer, mucous congestion and edema, erosion and hemorrhage, also with the findings of histopathologic morphologic changes, inflammatory cell infiltration, caliciform cell decreasing and crypt abscess.The JCL can correct the abnormal changes of T cell subset effectively and decrease IgM, IgG level in peripheral blood, strengthen the vitality of NK cell. Its effects better than SASP, compared with the controls.This study took twonitro-benzene ways to make animal modeland used some experimental ways sueh as pathology, immunology tomade study. We took thorough study about mechanism of diseases from pathologic form, change rate of TLR, IgG, IgA, NK cell's vitality, examination of IFN level. We used JCL to treat experimental uc. The result suggested that JCL can increase the level of LTR and decrease the level of IgG and IgA. Its effects better thanSASP(P<0.05) JCL can strengthen the vitality of NK, increase Ic-I, LFN level. Its effects better than SASP(P<0.01).This study also analyses the pathogenesis of us we think that inthe onset of UC. Spleen-asthenia is the root and the wetness-heat is the branch. These lead to asthenia of both spleen and kidney, deficiency of both vital energy and yin at last. So the fundamental of therapy of UC is relieving the primary and secondary symptoms at the same time treating cold-syndrome and heat syndrome contemporaneously.The JCL have the effects of clearing away heat-evil and wetness-evil, invigorating the spleen and kidney, and astringent therapy.
