The prominent pathological feature in obstructive nephropathy is tubulointerstitial fibrosis. Many studies show that transforming growth factor beta-1 (TGF-betal) is the pivotal cytokine inducing the occurrence and progression of the tubulointerstitial fibrosis, especially in animal studies. We are aim to evaluate the relationship between TGF-betal and tubulointerstitial fibrosis, via detecting the expression of TGF-betal in the impaired kidney tissue and its concentration in the urine. Also we detect the expression of Intercellular adhesion molecule-1(ICAM-1), Proliferating cell nuclear antigen(PCNA) and Collagen type IV(Col-IV), which involve in the fibrotic process.Part I Immunohistochemical Study on the expression of TGF-betal, ICAM-1,PCNA and Col-IV in the obstructive kidney tissueObjectives To approach the expression of TGF-betal, ICAM-1, PCNA and Col-IV in the Obstructive kidney tissue, and to investigate their relationship with the tubulointerstitial fibrosis, in order to afford a possible way to reverse the fibrotic process in the future.Methods Thirty-three obstructed kidney(OBKs) tissues were from the paraffin-embeded sections after hydronephrosis operation. Thirteen control normal kidney(CNKs) tissue were from who went nephronectomy or the normal sites 5cm far from the renal cell carcinoma. In light of the degree of hydronephrosis, 33 cases OBKs were divided into two groups: moderate(18 cases) and severe(15 cases) hydronephrosis. And in terms of reasons leading to hydronephrosis, 33 cases OBKs were also split into two group: congenital(10 cases) and others(23 cases). HE stains along with Masson stains were to evaluate the degree of the tubulointerstitial fibrosis in the obstructive kidney tissues. And the expression of TGF-betal, ICAM-1, PCNA and Collagen type IV in the kidney tissue was measured by immunohistochemistry S-P method. The mean luminosity value and the positive ratio weredetermined by photo-analysis technique. All the results were showed by mean value ?SEM. Results (1) HE and Masson stains showed expanded or slightly atrophic cortical tubules, enlarged interstitial space and a great amount of inflammation cells infiltration in moderate hydronephrosis. In severe hydrophrotic tissue, there were marked atrophic renal tubules, thickened tubule basement membranes, marked enlarged interstitial space, proliferative fibroblasts, depositive collagens and even sclerotic glomeruli. According to the degree of tubulointerstitial fibrosis, CNKs had 2 cases in Grade I and others in Grade 0, while OBKs had 12 cases in Grade I, 14 cases in Grade II and 7 cases in Grade III. (2) In contrast to slightly positive in a few tubules and glomeruli in CNKs, TGF-betal strongly positive express mainly in the cytoplasma of proximal tubule in OBKs but negative in the interstitium. The mean luminosity of OBKs and CNKs were 0.2370 + 0.0127 and 1502 + 0.0156 respectively. There were significant difference between this two group(t= 19.501, P<0.01). The positive expression correlated with the degree of tubulointerstitial fibrosis(r=0.793, t=7.521, P<0.01). The mean luminosity in severe hydronephrotic group was significant higher than that in moderate group.( t=2.699, P<0.05). And there was no statistical significance between congenital group and other reasons group(t=0.111, P>0.05). (3) ICAM-1 was also found positive expression in the cytoplasma of the proximal tubule both in CNKs and OBKs, but the expression in CNKs was merely slightly while that of OBKs was strongly. The mean luminosity of OBK and CNK were 0.1783 + 0.0165 and 0.1405 + 0.0120 respectively, There were significant difference between this two group(t=7.480, P<0.01). And there were no statistical correlation between the expression of TGF-betal and ICAM-1. The expression of ICAM-1 also weren't correlated with the degree of tubulointerstitial fibrosis. Severe group and moderate group showed no significant difference(t= 1.223, P>0.05). And congenital group and other reasons group also had no significant differ...
