Changes Of The Clinical Effect And Machanism Of Myocardial Energy Metabolism By Electroacupuncture,Compound Danshen Dripping Pills And Acupuncture Medication Combined In Myocardial Ischemia

Objective: To compare the effect of Myocardial ischemia (MI) with Coronary Heart Disease(CHD) treated by Electroacupuncture(EA), Compound Danshen Dripping Pills(DSP) and EA-DSP Combined, meanwhile, combined the experiment to observe changes of coronary artery adjusted by myocardial energy metabolism, then found out the best treated-method of traditional Chinese medicine and proved the adjusted effect for myocardial ischemia of CHD by electroacupuncture-PC6 of the hand-jueyin pericardium meridian.Methods : In clinic, the changes of the ischemia ST-T were recorded by electrocardiogram(ECG) and dynamic electrocardiogram(Holter). The levels of endothelin(ET) and Calcitonin gene-related peptide(cGRP) were measured by inequlity method. The levels of cholesterol and triglyceride were measured by PAP method. The levels of high density lipoprotein cholesterol(HDL-C)and low density lipoprotein cholesterol(LDL-C) were measured by direct method. The levels of apolipoprotein A-I & B were measured by the immunoturbidimetry method. In experiment, we successed to copy the classical animal model of myocardial ischemia and reperfusion injury in Wister rats.Before myocardial ischemia and after MI 20 minutes and 40 minutes and perfusing 40 minutes and 60 minutes,the changes of the ischemia STu were recorded by ECG.The rats' cAMP and cGMP were measured by radioimmunoassay(RJA).The levels of ATP, ADP, AMP and adenosine were analyzed by high performance liquid chromatography (HPLC). The changes of microstructure of pathomorphology were observed by light microcopy. Thechanges of microstructure were observed by electron microcopy.Result: 1.A11 of the three kinds of methods can relieve angina pectoris,decrease Nitroglycerin and myocardial oxygen consumption and clinical situation integral, ameliorate ischemia ST-T of ECG and Holter, decrease the ET's levels and increase the cGRP's levels, decrease CHO, TG, LDL-C and ApoB, increase HDL-C. Apo A-I and ApoA-I/ApoB. Compared with the three groups, the difference was significant (P < 0.05 or 0.01 ) ,especially acupuncture-medicine group brought about the optimal effect. 2. The microstructure of ischemia of myocardium and pathomorphology's structure were enhanced to a difference extent after treating by the three kinds of methods - EA, DSP and EA-DSP in all the wister rats with experimentalmyocardial ischemia. And the three methods can also decrease the STu's elevation in ECG, the levels of cAMP and ADP* AMP> adenosine in rats' blood serum, increase the levels of cGMP , ATP in rats' blood serum. It's showed that the effect of three groups, especially EA-DSP group brought about the optimal effect. Compared with the groups, the difference was significant (P < 0.05 or P < 0.01) .Conclusion: 1. The effect of EA-DSP treating myocardial ischemia of CHD is more than of EA and DSP treating MI in clinic. 2. All of three methods can improve myocardial energy metabolism in experiment. Compared with the effect in clinic and experiment, EA-DSP group brought about the optimal effect, EA-DSP group >EA group > DSP group. 3. Electroacupuncture-PC6 of the hand-jueyin pericardium meridian has the satisfactory adjusted effect to myocardial ischemia of CHD.
