[Background and Objective]Hepatic fibrosis which is associated with the production and deposition of extracellular matrix is a pathological response to various chronic liver diseases. Although hepatic fibrosis is thought to be a reversible pathological state, there is no established therapy, for hepatic fibrosis. Several traditional Chinese herbs have shown a promising future on protection of hepatic function in some studies. This study aimed to evalute the effects of musuwan on experimental hepatic fibrosis in SD rats and patiens with liver cirrhosis. By detecting liver function, serum fibrosis-associated enzymes, liver histopathological changes and mRNA expressions of MMP-2, TIMP-1, the exact effects of musuwan on rats of experimental hepatic fibrosis and its potential mechanisms may be concluded.[Materials and Methods) Part One. Effects of Musuwan on Experimental Hepatic Fibrosis in Rat.The SD rats were divided randomly into: normal group, model group, musuwan groups (small dose group, middle dose group and large dose group) and ruanganpian group. Hepatic fibrosis model was induced by injction DMN, 1 ml/kg body weight, peritoneal injectoin, three times weekly, three weeks. Rats in prevention group were given drugs at first injection of DMN. Rats in treatment group were given drugs after injection of DMN. At the end of the experiment, all the rats were killed. The rat blood was withdrawn from the heart and the livers were removed. Examinations of serum aminotransferase, ALP, PT, Alb, HA, LN were performed, at the same time, tissue sections either were stained with HE, VG method or subjectedto immunohistostaining by using antibodies against a -SMA.Histopathologic changes and extra-structure of the rat's liver were observed. Part Tow. The Mechanism of Musuwan on Rat Liver Fibrosis.Serum Surperoxide activities and Malondialdehyde were observed.Immunochemistry technic and RT-PCR were used for analysis theexpression of MMP-2 , TIMP-1.Part Three. Effects of Musuwan on Patiens with Liver Cirrhosis.The information of each patiens' complications and symptoms were collected. In order to reseach the treatment effect of musuwan on patiens with liver cirrhosis, serum liver function was observed on the lth, 2th and 3th month after the first musuwan treatment respectively.[Results]Part One. DMN injection could induce rat hepatic fibrosis. All the rats had hepatic fibrosis in the end. Part Two. Effects of Musuwan on Experimental Hepatic Fibrosis in Rat.The elevated serum ALP activities were markedly decreased, and the decreased Alb was improved with musuwan treatment and ruanganpian trenament. The elevated serum HA, LN were markedly decreased in musuwan middle dose and large dose preventing groups and ruanganpian preventing group. The pathology change of collagenic fibers in liver was alleviated in musuwan middle and large dose prevention groups. The severity of fibrosis was not significantly decreased in musuwan little dose preventing group and musuwan treatment groups compared with DMN group. Part Three. The Mechanism of Musuwan on Rat Hpatic Fibrosis.The serum MDA in the treatment with musuwan was obviouslyreduced. The decreased SOD activities were improved with musuwantreatment.
