Objective To explore the effects of the Rennin angiotensin system (RAS) related gene polymorphisms, including Angiotensin converting enzyme (ACE) Insertion/Deletion(I/D),Angiotensin II type 1 receptor (AT1R)( A1166C) on liver cirrhosis by investigating the correlation between ACE(I/D) and AT1R(A1166C) gene and hereditary susceptibility and clinical phenotype of liver cirrhosis induced by HBV infection, to find susceptibility gene, to filter the high risk group susceptibility to liver cirrhosis and to reduce the incidence and mortality of the liver cirrhosis.Method Peripheral blood samples were collected from 72 patients with HBV induced liver cirrhosis,which were further classified into three groups (A, B and C groups) according to Child Pugh classification and 46 healthy blood donors.The polymorphisms of ACE(I/D) and Al166C of AT1R gene were determined by PCR-RFLP, and the plasma concentrations of angiotensin II were measured by RIA.Results The plasma concentrations of angiotensin II of the group of liver cirrhosis was significantly higher than those of the control group (P<0.05). In Child Pugh C , B grade group the plasma concentrations of angiotensin II was significantly higher than that of Child Pugh A grade group (P<0.01,P<0.05, respectively), there was no significant difference for ACE (I/D) polymorphism between the cirrhosis group and the control group (P>0.05), and the frequency of II genotype is higher in portal hypertension group and in ascites group with liver cirrhosis than without portal hypertension and ascites (P<0.05);there was no significant difference in genotypes and allele frequency of AT1R(A1166C) between the liver cirrhosisgroup and the control group (P>0.05).Conclusion There had relationship between The plasma concentrations of angiotensin II and severity of liver cirrhosis, there had no relationship between the A1166C genopolymorphism of AT1R gene and hereditary susceptibility to liver cirrhosis,and there had relationship between the frequency of II genotype of ACE (I/D) and the occurring of liver cirrhosis, and the occurring of portal hypertension or ascites. ACE (I/D) polymorphism may play a significant role in development of liver cirrhosis, but has no correlation with the A1166C polymorphism of AT1R gene and progression of liver cirrhosis.
