Objective: To investigate the change and significance of LPS and its recognition receptors CD14, TLR4 in prophylactic treatment for multiple organ dysfunction syndrome with high volume hemofiltration. Methods: 28 pigs were divided randomly into three groups (group C, group M, group HF), major organs functions were monitored, the concentrations of plasma and ultrafiltration TNFa , IL-10, IL-6 and LPS were measured, the changes of monocytes CD14, TLR4 protein and mRNA in peripheral blood were detected. Results: Earlier high volume hemofiltration could cut down the plasma concentrations of inflammatory cytokines, obviously improve the functions of major organs, reduce the MODS morbidity and mortality, cut down the plasma concentrations of LPS and down-regulate the expression of monocytes CD14, TLR4. Conclusions: The change of LPS and its receptors CD14, TLR4 may play an important role in the pathogenesis of MODS. Earlier HVHF could remove the high concentration of blood LPS and correct the unbalanced relation between pro-inflammatory and anti-inflammatory effectly and down-regulate the expression of CD14, TLR4, thereby inhibit the development of uncontrolled inflammation at earlier period and the development of MODS.
