BackgroundDiabetes and hypertension are both major risk factors for cardiovascular diseases. The prevalence of hypertension is increased in the diabetic population, and hypertension associated with diabetes further increases the risk of cardiovascular morbility and mortality. It is important to explore deeply the machanism and the methods to block the its development in hypertension associated with diabetes.The renin-angiotensin system(RAS) plays a major role in the development of diabetes and hypertension. Recently, however, the classical view of the RAS has been renewed by the discovery of the angiotensin-converting enzyme 2(ACE2). ACE2 degrades angiotensin â…¡ (Ang â…¡) to the biologically active peptide, angiotensin-(1-7) [Ang-(1-7)I. It is not clear that the mechanism of ACE2 counteract ACE in hypertension, heart failure and diabetes.Apelin is an important physiological regulator. It is the ligand of putative receptor protein related to the angiotensin receptor AT1(APJ). It is the ACE2 substrate. Apelin and APJ have proved to be important regulators of the renal structure and function of hypertension, diabetes. Further studies are needed to determine the functions of apelin and APJ.Many questions remain to be answered about impacts of ACE2, apelin and APJ.
