Research On Intervention Of Nourishing Yin And Promoting Blood Circulation In Ventricular Remodeling

In recent years, it is gradually understood that ventricular remodeling (VR) is the main pathophysiological change in heart failure, in both west medicine and Chinese medicine, VR is key point in treating heart failure. Based on the achievements of modern medicine and Chinese medicine, the article makes systematic studies on intervention of nourishing Yin and promoting blood circulation in VR through theoretical, experimental and clinical studies.In theoretical studies, the article introduces the improvement of VR in modern medicine, and elucidates the mechanisms of renin-angiotensin-aldosterone system (RAAS) , aldosterone and mineralocorticoid receptor in VR. According to the achievements of VR in Chinese medicine, the article also put forward that the main pathogenesis in VR is deficiency in Ben and excess in Biao, Yin deficiency and blood stasis, deficiency in Ben causing excess syndromes and serving each other; the therapeutic rules is to treat Biao and Ben at same time, and therapeutic method is to nourish Yin and promote blood circulation.In experimental studies, we reproduced the transgene mouse models of mineralocorticoid receptor, and tested several indications in VR, the results showed that aldosterone performs an important role in VR and estrogen has effect on inhibiting VR. In experiment, the results confirmed that compound prescription of Chinese Medicine have effect on restricting VR, the mechanism of which may be related to anti-aldosterone.In clinical studies, 36 patients with heart function in I～II degree of chronic heart failure were divided into two groups, 21 cases in therapeutic group and 15 cases in control group, all patients were given routine treatment. In addition, gave prescription of nourishing Yin and promoting blood circulation to therapeutic group, and Spironolactone tablets to control group. The results showed both west medicine and herbs can improve the indications of VR, and the mechanism of inhibiting VR by Nourishing Yin and promoting blood circulation may be related to anti-aldosterone.
