The local pathological changes of liver is the common imaging of liverdisease, the aim of imaging examination is to diagnose the benign andmalignant disease. Now, it is the most valuable that the expression ofmorphology combine with real-time gray-scale contrast-enhancedultrasonography, and it is best way to study and use in the clinical medicine.Because the blood supply of malignant tumor in quality and quantity isdifferent from benign disease significantly, so evaluation of hemodynamicalcharacter by contrast enhancement methods without injury is becoming thehotspot . When volume of malignant tumor exceeds a certain range, it willstimulate the growth of blood vessel. New blood vessel not only can providenutrition of tumor but also contribute to tumor metastasis, it is play animportant role in growth and metastasis of malignant tumor. The tumorangiogenosis is a complicated process, its completion is mediated by tumoritself and cell factor secreted by tuomr related inflammatory cell. Vascularendothelial growth factor(VEGF) is known as a vascular permeability factorand plays an important role in tumor angiogenosis. Literature reported that thepeak attenuation of tumor in CT reflects intratumoral microvesseldensities(MVDs).In this study, we analyze the function of VEGF andintratumoral microvessel densities(MVDs), evaluate the expression of VEGFand MVDs in primary hepatic carcinoma, discuss the correlation betweenenhancement of gray-scale contrast-enhanced ultrasonography in hepaticcarcinoma and tumor angiogenosis.Partâ…  Diagnosis of focal hepatic lesions with real-time gray-scalecontrast-enhanced ultrasonographyObjective:To study the new type contrast media Sono Vue in dynamicgray scale ultrasonography and evaluate its vale in differential diagnosis ofhepatic lesions.Methods:106 focal hepatic lesions were examinde with dynamic grayscale ultrasonography,all the cases were diagnosed by surgerry or needlebiopsy. 48 primary liver carcinomas, 21 metastases, 15 angiomas of liver, 8focal nodular hyperplasias of liver,8 cirrhosis of liver nods, 6 focal fatty livers,2 inflammatory pseudotumors of the liver, 1 tublerculosis of liver, 2 limitedfatty infiltrations were occupied.Results: 89.6%(43/48)of primary liver carcinomas enhanced early thanparenchymas of liver, at portal venous phase and delayed phase werehypoechioc, 86.7%(13/15)of liver angiomas at arterrial phase had nosignaficent enhanced,and then gradully filled insieds. Focal nodularhyperplasias of liver had a contrst-enhancement pattern of "fast-in andslow-out".At all phase liver inflammatory pseudotumors and tublerculosis ofliver had no enhancement. Enhancement of cirrhosis ofliver nods, focal fattylivers and limited fatty infiltration were same with parenchyma of liver .Manifestation of liver metastases were diversify, most annular enhanced atarterrial phase and portal venous phase,and the speed of wash out weredifferent.Conclusion: Contrast-enhanced dynamic gray scal ultrasonographydemonstrates is useful in the differential diagnosis of hepatic lesions.Part â…¡ The crrelation between real-time gray-scalecontrast-enhanced ultrasonography and tumors angiogenesis inhepatocellular carcinomaObjective: To study the corerelation between the contrast-enhancedultrasound patterns and histological differentiation, microvessel density andvascular endothelial growth factor(VEGF) expression in hepatocellularcarcinoma(HCC).Methods: The contrast-enhanced ultrasound examination was performedbefore operation in 38 patients with histopathology proven HCC. The dynamiccontrast-enhanced ultrasound patterns were recorded. Specimens were stainedwith hematoxylin eosin and in immunohistochemically for CD34 and VEGF.The contrast-enhanced ultrasound patterns and histological differentiationwere compared and analysed.Results: In 39 HCC patients with different histological differentiationgrades , the time of enhancement beginning had no obvious difference(ANOVA,P>0.05 ).Whereas the eanhancement duration and the time of ofenhancement fadeaway had obvious difference (ANOVA,P<0.05 ,P<0.05);histological differentiation grades of HCC have positive correlation to theeanhancement duration and the time of of enhancement fadeaway.(39 HCC patients were divided into 3 groups with the higher(>30),medium(20-30) and lower(<20) microvessel density.Among them,) In 39 HCCpatients from 3 groups with the higher , medium and lower microvesseldensity , the time of enhancement beginning had no obvious difference(ANOVA,P>0.05 ).Whereas the eanhancement duration the time ofenhancement fadeaway had obvious difference (ANOVA,0.010.05, P>0.05, P>0.05).Conclusion: The real-time ultrasound contrast patterns in HCC wereaffected/influenced by tumor angiogenesis. And it reflected tumor vascularhyperplasia in a certain extent.
