The indication for operation of congenital heart disease with pulmonary hypertension mainly depends on the severity of pulmonary vascular pathology and its possibility to recover. Although lung biopsy usually can define the severity of pulmonary vascular pathology and provide evidence for screening and estimating prognosis, it is an invasive method and cannot be used as routine procedure. Pulmonary artery pressure/systemic artery pressure has no relationship with pulmonary vascular resistance when inhalation of oxygen because oxygen inhalation will dilate pulmonary vessels and increase pulmonary blood flow/systemic blood flow~[1]. The side effect of vein dilator is hypotension and reduction of systemic vascular resistance, unbalance of ventilation/perfusion ratio and hypoxemi a So oxygen and vein dilators are not good enough to be used to evaluate pulmonary hypertension. On the contrary, inhalation of NO do not affect systemic vein oxygen saturation and has no complications as hypotension and hypoxemia~[3]. The pulmonary vascular resistance can be calculated accurately. NO is a valuable specific pulmonary vascular dilator in evaluating pulmonary hypertension.
