There are lots of study about the penumbra and its temporal window. The most of them are about animal; rat, mouse etc., about electrophysiology biochemistry and imaging changes (PET, functional CT, diffusion MRI etc.). And a few of them are about human autopsy neuropathology, few of them expound the criteria for penumbra partition. So we made a serial of study to look for the answer.Aim: To study the temporal MRI T2 imaging, T2 value, morphological changes of neurons, astrocyte, microglia, small blood vessel on large sections of whole brain and small sections by histochemisrey and immunohistochemistry method. Lots of immunohistochemistry targets were used, such as GFAP, CD68, MAP2, NF200, FAS, TNFalpha and TGFbeta1. TUNEL and FAS were used to different apoptosis at different time. Then to find the possibility for define penumbra by histology .At the same time, we also discussed the significance of dark neuron, cytoskeleton, microglia, astrocyte, apoptosis cell, macrogphage, TNFalpha, and TGFbeta of inflammation in cerebral infarction evolving.Material and method: Nine brain, seven of cerebral infarction and two of cerebral hemorrhage, who ages 60 to 67 years old, were included in our group. The ischemia time are ten hours, 27 hours, 48 hours, three and half day, seven days, twelve days, fourteen days. The suffered hemisphere was first attacked. Four of them had hypertension and/or diabetes. One of them have drinking and smoking history .All the nine patients have severely atherosclerotic or atherosis and/or thrombosis in average five to six parts of cerebral artery.Fixed by 10% formaldehyde one month to one year later we examined by MRI.
